    <jats:p>Although public interest in telemedicine is growing, limited research has examined the necessary conditions for its usage. Therefore, this study aims to identify these conditions in Japan and China.</jats:p>
    </jats:sec>
    <jats:sec>
    <jats:title>Methods:</jats:title>
    <jats:p>A self-developed questionnaire was administered between 2022 and 2023, yielding 787 valid responses in Japan and 840 in China. In addition, 241 Japanese and 194 Chinese supplemental responses were collected in 2025. Factor analysis and necessary condition analysis (NCA) were applied.</jats:p>
    </jats:sec>
    <jats:sec>
    <jats:title>Results:</jats:title>
    <jats:p>
    Based on the initial samples, eight factors were extracted: cost and accessibility, social influence, safety and reliability, adaptive readiness, technology confidence, perceived data security, facilitating conditions, and perceived usefulness. NCA revealed cross-national differences. Japanese participants mainly regarded perceived usefulness (
    <jats:italic toggle="yes">d</jats:italic>
    = 0.12–0.31) and facilitating conditions (
    <jats:italic toggle="yes">d</jats:italic>
    = 0.15–0.29) as necessary conditions, whereas Chinese participants perceived a broader set of prerequisites. Subgroup analyses further indicated that safety and reliability were the most critical necessary conditions for experienced users in both countries (Japan:
    <jats:italic toggle="yes">d</jats:italic>
    = 0.44; China:
    <jats:italic toggle="yes">d</jats:italic>
    = 0.28). Hierarchical clustering analysis further revealed three bottleneck trajectories of constraints: gradual-bottleneck users (progressive constraints), high-bottleneck users (sudden demand surges), and early-bottleneck users (persistent initial constraints). Crucially, the supplemental samples further confirmed the reliability and validity of the eight-factor structure and the temporal consistency of the necessary conditions for sustained usage.

    <jats:title>Conclusion:</jats:title>
    <jats:p>The findings highlight the need for tailored telemedicine strategies. In particular, maintaining safety and reliability represents a nonnegotiable prerequisite for sustained telemedicine usage in both countries. The developed instrument, combined with NCA, offers a transferable approach for identifying constraints and developing more adaptive and equitable telemedicine strategies.</jats:p>

    Medical institutions typically rely on manual analysis of adverse medical events, which requires significant human resources, time, and specialized knowledge and expertise. These requirements reduce the effectiveness of identifying potential risks. Can large language models (LLMs) leverage their powerful natural language processing capabilities to function as reliable risk detectors? In this pilot study, we aim to evaluate the effectiveness of LLMs in identifying electronic medical record system (EMR)-related medical incident risks. We first curated a dataset comprising 573 medical incident reports that had been manually analyzed. Then, using a few-shot prompting approach, we designed instructions to evaluate five LLMs, including GPT-4o, Claude 3.5 Sonnet, DeepSeek V3, Nova Pro, and Llama 3.1-405b. The results indicated that the best-performing LLMs could accurately extract more than half of the risk factors and generate reasonable explanations grounded in real-world case contexts. While general-purpose LLMs can provide some assistance, further optimization tailored to specific medical scenarios is necessary to enhance their capability in handling complex cases.

    <jats:p>Background: Advanced sensor technology, such as commercial wearable activity trackers, is expected to be useful for improving public health in an aging society. However, its effects on psychological and behavioral changes in older adults have not been thoroughly investigated. Objective: This study aimed to clarify the effects of wearable activity trackers on psychological and behavioral changes in older adults before the pandemic and to confirm whether the effects were maintained during the pandemic. Method: Thirty Japanese older adults wearing Fitbit trackers for 12 weeks (i.e., six weeks before the pandemic and six weeks during the onset of the pandemic) in a real-life setting participated in this study. Participants’ perceptions of physical activity (PA) and monitoring were measured weekly using a questionnaire in addition to the pre-monitoring survey (baseline). Results: Significant positive effects of activity trackers were observed on participants’ awareness of the benefits of PA and motivation toward monitoring from the first week, and high levels were maintained until the end of the study, regardless of the pandemic. Accordingly, an approximately 3,000-steps/day increase was observed in the first week and maintained before the pandemic; although PA levels decreased during the onset of the pandemic, they remained higher than at baseline. The decrease in the number of steps and activity calories in COVID-19-non-affected participants was 2%. In the affected participants, an 11% decrease in steps and an 8% decrease in activity calories were observed, indicating that 3% was compensated by other activities. Conclusion: Positive effects of activity trackers were observed on older adults’ awareness of PA benefits. Thus, future studies are required to verify the effects of increased PA benefits awareness on older adults’ long-term activity levels for their health.</jats:p>

    In FY 2020, based on the analysis results of EMR-related incident records by applying the developed taxonomy system, characteristics of these incidents have been explored. For instance, the frequent failures were because of healthcare staff’s wrong input, missing input and lacking of data check (patient data in EMR). Latent factors behind these failures included insufficient training, interruptions, varied culture between different units/departments, etc. Then, characteristics of EMR-related incidents have been almost confirmed by a questionnaire survey followed by interviews to healthcare professionals. Then, EMR-related patient safety management experience was learned from the EMR advanced countries as Norway and Denmark. Based on all the findings and advanced experience, a draft of EMR-related incident management system has been developed. It includes contents related to incident reporting, i.e., what kind of incidents should be reported, to what extent and how. Thus, a common format for EMR-related incident reporting was designed. The system also includes how to make analysis of incidents; corresponding to the analysis results, what kind of measures could be taken and how; and how to make evaluations after taking measures.

  • 高齢者生活・健康情報の自動収集・分析フィードバックの効果：社会・人間特性複合評価

    ①高齢者の生体・健康情報の自動収集・フィードバック・システムの利用効果の実証的評価: 歩行ステップ数、睡眠時間、心拍数などの日常活動・生体情報を自動収集し、結果をスマートホンにフィードバックするシステムを、高齢者が自分自身の生活環境で長期間継続的に使用したときに、健康状態の推移とともに、健康や日常的運動に対する意識・モチベーション・満足度・不安といった心理・精神状態に、どのような効果があるのかを数名の高齢者を利用して調査した。調査を実施した高齢者概ね良好な高価が見られた。特に、歩行などの日常活動への意欲、健康・生体情報の継続的計測など、心理面・精神面への好影響が見られた。
    ②若年者の生活・健康情報の自動収集・フィードバック・システムの利用効果の実証的評価： 文献サーベイから高齢時の運動・日常活動の習慣をつけるには、若い時期からこのような活動の習慣が重要であることが示唆されていることがわかった。これに従って40名以上の若年被験者を対象に3ヶ月間の日常生活中での生体・健康情報の自動収集、その分析結果のフィードバックを行い、高齢者対象のトライアルと同様に、健康状態の推移とともに、健康や日常的運動に対する意識・モチベーション・満足度・不安といった心理・精神状態を調査するトライアルを実施した。若年者についても生体センサーをつけた健康状態等の継続的測定とフィードバックにより概ね良好な心理的効果は見られたが、否定的影響を受ける被験者も存在し、個人差がかなりあることがわかった。
    ③高齢者の生活・健康情報フィードバックの医療者・介護者への影響分析: 数人の医師、介護者に対して、このシステムの利用による彼らの職務への影響に関するインタビュー調査を実施した。これにより彼らの職務に対するメリットが大きいことがわかったが、生体センサーへの充電の問題など、実践上の問題点もいくつか示唆された。

    本研究では、医療事故の低減に眼球注視技術を役立たせる応用技術の開発を行い、現実の医療業務に対する適用を通して、この技術の有効性、および本格利用に対する将来展望を明らかにする。具体的には、安価で非侵襲な眼球注視測定装置を利用して、業務中に医療者が意識することなく、エラー解析に有効な視覚データを自動収集でき、そのデータと成功ケースの効果的マッチングにより医療業務中の医療者のエラーを自動検出できる眼球注視応用技術を開発した。このシステムを、電子カルテを用いた医師の薬剤処方、および人工透析装置の操作に適用し、その有用性、妥当性の評価を行った。

    本研究は患者移送の円滑な実施を阻害する要因であるコミュニケーション不足などの問題に焦点をあて、安全で効果的な組織文化およびプロトコルの構築を目指した。様々な研究方法をアプローチし（例えば、アンケート調査、インタビュー、エラー分類法によるインシデント報告の分析など）、主な研究成果は以下になる：（1）病院内の部門間（診療科・病棟）、および看護師のシフト交代時の患者移送の組織文化、現状と問題点を明らかにした；（2）移送元、移送先、患者間における効果的なコミュニケーションを可能とする標準的なプロトコルを構築した;（３）患者移送の合理的な評価が出来るツールを設計し、構築したプロトコルの有効性を検証した。
