    BACKGROUND: Coronavirus disease 2019 disrupted the delivery of public maternal and child health services to caregivers of preschool children, leading to decreased opportunities for injury prevention education. We aim to 1) explore the timing, content, and methods of providing injury prevention information desired by pregnant women and mothers and 2) identify mothers who experienced difficulty in obtaining injury prevention information owing to reduced maternal and child health services. METHODS: From March 24 to 29, 2022, we conducted a population-based cross-sectional study and web-based survey. Of the registered monitors of the internet research company Rakuten Insight, 675 mothers raising their first child aged 0-2 during the COVID-19 period (February 2020 to March 2022) were included in the analysis. RESULTS: Over half of the mothers wanted injury prevention information throughout their pregnancy. They preferred receiving information through traditional face-to-face services provided by local governments, such as antenatal classes or checkups. However, 34.1% of mothers said they did not obtain the information they needed; this was particularly true of unemployed mothers, had children aged 0-1, and had children with illnesses requiring hospital visits. CONCLUSIONS: Mothers who could not obtain injury prevention information were originally disadvantaged mothers concerning access to information. The decrease in maternal and child health services may have widened this information gap. These findings can inform recommendations for caregivers, particularly those susceptible to information gaps during emergencies, and offer insights into future injury prevention education strategies.

    This study examined the long-term effects of an infant injury prevention program implemented during an antenatal class of 131 mothers. Questionnaires were completed 2 years postpartum to assess the incidence of injury (medically attended or home-care), mothers’ perception of injury prevention, implementation of safety practices, and active attitudes toward injury prevention. Responses were obtained from 68 (51.9%) mothers (intervention group, 40; control group, 28), including 24 who reported medically attended injuries and 55 who reported home-care injuries. The incidence of medically attended injuries did not differ between groups. The incidence of home-care injuries was also not significantly different, but was lower in the intervention group (72.5% vs. 92.9%, p = 0.050). Significantly fewer children in the intervention group experienced “injury due to being caught between objects” (12.5% vs. 39.3%, p = 0.014). Mothers in the intervention group were significantly more aware of injury prevention than those in the control group (p = 0.033). The risk of home-care injuries was inversely related to mothers’ injury-prevention perception (odds ratio [OR]: 0.55, p = 0.035). This study suggests that group education during pregnancy regarding injury prevention increases mothers’ perception of injury prevention. These findings support implementing injury prevention education during antenatal classes.

    OBJECTIVE: This study aimed to elucidate the experiences of public health nurses (PHNs) in Japan during the first wave of COVID-19. DESIGN AND SAMPLE: Twelve PHNs in charge of responding to COVID-19 in X-city within Tokyo metropolis in Japan participated in this case study. MEASUREMENTS: Data were collected through self-administered questionnaires and semi-structured interviews on PHNs' experiences from January 2020 to May 2020. RESULTS: Initially, only infectious disease control division (IDCD) PHNs experienced confusion due to the rapidly increased workload. Managerial PHNs attempted to explain the need for a dispatch system for the IDCD, using available statistical data from other managerial members, within one's maximum understanding of this unprecedented situation. Without having a clear and forward-looking understanding regarding the purpose and reasons for dispatching, some dispatched PHNs had concerns and frustrations; they did not view the COVID-19 pandemic as a disaster. In the never-ending, exhausting work, PHNs managed to modify the provision of conventional services to residents. CONCLUSIONS: Despite experiencing confusion, PHNs worked to continuously provide community services, re-considering the meaning of public health nursing. Prioritizing the work and shifting tasks to other professionals at an early stage of the pandemic may prevent organizational dysfunction.

    <jats:p>It is important to educate caregivers in order to prevent infant injuries. However, there have been few studies on the effects of education on pregnant women. This study aimed to evaluate the effects of injury prevention group education on this group. Study participants were recruited from a group of pregnant mothers attending an antenatal class in Tokyo. Participants were assigned to either the intervention or control group based on the month in which they attended the existing antenatal class. Both groups received a leaflet on injury prevention, but only the intervention group received an additional short one-shot lecture. The implementation of each of the nine safety practices was assessed during home visits after childbirth. Of the 131 study participants (56 in the control group and 75 in the intervention group), 106 (80.9%) received home visits after birth. Mothers in the intervention group implemented three practices significantly more than those in the control group: Keep soft objects away from the baby’s head (38.3% vs. 13.0%), Do not place your baby on a high surface (74.6% vs. 52.2%), and Use the baby carrier correctly (93.3% vs. 76.1%). In the future, we plan to follow up the participants to evaluate the program’s long-term effects, and to continue to improve the program.</jats:p>

    <jats:p>Injury prevention education for pregnant women may be beneficial for infants’ safety. Currently, knowledge about the scope of an expectant mother’s intent to prevent injury is limited. The objective of this study was to determine pregnant women’s intentions to implement infant injury prevention strategies. From May to June 2017, a self-administered questionnaire based on the precaution adoption process model was distributed among pregnant women who participated in a parenting preparation class in a city, Tokyo. Pregnant women’s intentions to implement the following eight kinds of safety practices were measured: three practices regarding suffocation, two regarding falls, one safety practice for burns, one for accidental ingestion, and one for traffic accidents. Among 132 respondents (response rate: 83.5%; mean age: 33.4 years; mean gestational age: 29 weeks), the most common unawareness issue was “Make sure that there is no space between the mattress and bed frame” (68.2%), followed by “Use a firm mattress or futon” (38.5%) and “Keep soft objects away from the baby’s head in the baby’s sleep area” (31.8%); 58% or more women reported having already “decided to implement” the other five practices. Safety practices that pregnant women were mostly unaware of were for preventing suffocation, despite this being a leading cause of death in terms of unintentional infant injury. In comparison, the safety practices for falls, burns, and accidental ingestion were more known to pregnant women. The pregnant women’s intention to implement injury prevention for infants varied by safety practices. These findings could be used to improve the focus of antenatal education programs for the prevention of infant injury.</jats:p>

    BACKGROUND: Developmental dysplasia of the hip remains a preventable yet underdiagnosed condition. Conventional screening strategies that rely on physical examination demonstrate limited sensitivity and fail to achieve comprehensive population coverage. Although universal ultrasound screening has shown promise, evidence supporting scalable, sustainable, and nurse-led community-based implementation models within real-world public health systems remains limited. OBJECTIVE: To evaluate a nurse-led community-based ultrasound hip screening program integrated into routine maternal and child health services in Japan using the Reach, Effectiveness, Adoption, Implementation, Maintenance framework. DESIGN: A prospective cohort study with a mixed-methods design. SETTINGS: Three rural Japanese municipalities (February 2024 to August 2025). PARTICIPANTS: Public health nurses, midwives, infants, and their caregivers who underwent routine newborn home visits or child-rearing consultations in the participating municipalities. METHODS: The existing maternal and child health services evaluated across the following dimensions: Reach (coverage rate); Effectiveness (the proportion and mean age of infants referred early to medical institutions, and caregiver-reported outcomes following referral); Adoption (nurse adoption rate); Implementation (the proportion of ultrasound images meeting Graf's criteria, examination time, operational challenges, and cost); and Maintenance (continuation and policy integration). Ultrasound images were uploaded to a secure encrypted cloud server and interpreted remotely by Graf-certified pediatric orthopedic surgeons, with structured feedback provided to nurses throughout the implementation process. RESULTS: A total of 818 examinations were performed in 349 infants. The estimated coverage rate was 95.6%, indicating a near-universal range. Standard-plane images meeting Graf's criteria were obtained in 85.8% of examinations. Non-Type I hips accounted for 8.7% of cases, and 7.0% of infants were referred for further evaluation. Of these referred infants, 97.6% visited medical institutions, 54.8% required continued follow-up, and 4.8% required treatment. Adoption was high, with 95.5% of trained nurses conducting screenings. Implementation challenges included infant positioning, data-upload errors, and limited digital integration; however, interprofessional feedback from orthopedic surgeons improved technical performance. Ultrasound visualization enhanced caregiver understanding and facilitated timely adherence to referral recommendations. All municipalities continued screening, demonstrating strong maintenance. CONCLUSIONS: Nurse-led ultrasound hip screening can be feasibly integrated into routine maternal and child healthcare systems. This model enables early identification of suspected developmental dysplasia of the hip without clinical signs, and offers an equitable approach in resource-limited settings from a precision public health nursing perspective.     OBJECTIVE: Physical assessment by public health nurses is vital for childhood health, but its education remains unclear. This study examines the structure, process, and outcomes of physical assessment education for newborn and infant home visits in public health nursing schools. DESIGN: Nationwide cross-sectional study. SAMPLE: Of the 280 public health nursing schools in Japan, 85 were analyzed. MEASUREMENTS: Survey based on Donabedian's model: structure (e.g., program type), process (e.g., practice-oriented education score), and outcome (e.g., proportion of graduates who achieved the graduation goal). Descriptive statistics and regression analysis were conducted. RESULTS: Although 69.4% of schools rated physical assessment education as "very important," graduation goals varied substantially: 11.8% targeted "Does independently with minimal guidance," 32.9% "Does under supervision," 50.6% "Shows," and 4.7% "Knows." Practice-oriented education scores ranged widely across schools (9-80). Higher graduation goal levels were significantly associated with greater practice-oriented education (B = 5.669, p = 0.024). Greater practice-oriented education was positively associated with higher proportions of graduates achieving graduation goals (B = 0.029, p = 0.042). CONCLUSIONS: Substantial variability exists in the structure and delivery of physical assessment education for newborn and infant home visits. Strengthening practice-oriented, competency-based education may support more consistent competency development in foundational education.     BACKGROUND: Conventional physical examinations sometimes fail to detect developmental hip dysplasia. Ultrasound hip screening, non-invasive and radiation-free, can identify these cases earlier, and nurse-led maternal, newborn, and infant home visits and childcare consultations at community health centers offer an ideal platform for community implementation. Knowing the relationship between Objective Structured Clinical Examination scores and nurses' ability to capture diagnostic-quality hip images would inform training standards and credentialing, ensuring safe scale-up of nurse-led hip-screening services. However, this link remains unknown. OBJECTIVE: To examine the association between Objective Structured Clinical Examination performance and nurses' success in capturing standard hip ultrasound images during maternal and child health service consultations. DESIGN: Prospective cohort study. SETTINGS: Three municipalities in Japan. PARTICIPANTS: The study included 21 nurses (18 public health nurses, 1 registered nurse, and 2 midwives). METHODS: Participants completed an e-learning course, knowledge tests, hands-on training seminars, and the Objective Structured Clinical Examination before undergoing ultrasound examinations during home visits. Two trained researchers assessed exam performance using a global rating scale (range: 1-6), total score (range: 0-360 points), image acquisition time, and successful capture of standard images of the right and left hips using phantoms-infant-shaped models for training ultrasound hip screening. Pediatric orthopedic surgeons evaluated the ultrasound images obtained during home visits between February 2024 and May 2025. Linear regression analysis examined the associations among participant demographics, examination performance, and imaging success rates. RESULTS: The mean examination global rating score was 4.48 (standard deviation = 0.66), and the mean total score was 330.8 (standard deviation = 22.0). During home visits, 611 ultrasound examinations were conducted, of which 494 (80.9%) were successful. The success rate did not vary substantially based on the cumulative number of examinations performed by each nurse. Lower age (B = -5.2, p = 0.030) and successfully capturing a standard plane of the left hip during the examination were associated with significantly higher imaging success rates in maternal and child health service consultations (84.7% vs. 71.4%, p = 0.039). A shorter image acquisition time (B = -0.1, p = 0.009) was also significantly associated with higher success rates. CONCLUSIONS: Successful capture of left-hip image and faster performance during the Objective Structured Clinical Examination independently predicted nurses' field imaging success, supporting this examination as a readiness checkpoint and guide for targeted remediation.     AIMS: Early detection of developmental dysplasia of the hip in newborns and infants is essential to ensure lifelong mobility. As several municipalities conduct nurse-led hip ultrasound screenings during newborn and infant home visits, nurses must acquire the accurate technique smoothly and effectively. Clarifying the challenges nurses face when performing ultrasound procedures helps support effective education. This study aimed to identify challenges inexperienced nurses face when performing basic ultrasound using the Graf-method in home settings. DESIGN: Descriptive case study. METHODS: Ultrasound training was provided to 22 nurses in Japan to enable use during newborn and infant home visits. We analyzed 25 video cases from three nurses who required > 180 s to obtain a standard plane image. Videos were transcribed and inductively analyzed to identify challenges with reference to Yin's case study methodology. RESULTS: Conditions of the challenges were categorized into knowledge, skills, and attitudes with reference to Bloom's taxonomy. The knowledge gap led to the failure to correctly judge whether the femoral head/standard plane was captured. Skill deficiencies led to failure to move the probe vertically and difficulty maintaining the infant's straight lateral position. The combination of inadequate knowledge and skills resulted in failures to (1) place the probe directly over the femoral head, (2) display the image in the correct position and orientation, and (3) move the probe correctly regarding direction and range while maintaining the image in the correct position and orientation. Regarding attitude gaps, nurses failed to improve the procedure. CONCLUSIONS: Improving ultrasound proficiency for developmental hip dysplasia detection requires training in anatomy integration, sensory experience, and probe-screen coordination while addressing posture, equipment handling, and motor skills.     AIM: To evaluate the effect of satogaeri, a Japanese custom of mothers returning to their parents' home for perinatal and postnatal support, on the health statuses of mothers and infants and the child-rearing support from the surrounding environment. METHODS: We conducted a retrospective cohort study of all pregnancies reported at a city in Tokyo in 2020. Propensity score matching was performed between expectant mothers with the intention to have satogaeri (satogaeri group) and those without (residence group), with age, birth order, place of their parents' residence, and other psychosocial conditions at pregnancy reporting as variables. Follow-up data on mothers and infants from the newborns' 4-month checkup were compared. RESULTS: We analyzed 1559 expectant mothers, including 399 who had satogaeri. Follow-up data of 270 and 314 in the satogaeri and residence groups, respectively, were compared after matching. Satogaeri was associated with neither infants' 4-month checkup results, including pediatrician assessments (no abnormality: satogaeri group 54% vs. residence group 46%, p = 0.342), nor mothers' health conditions, including perceptions of parenting difficulties (12% vs. 13%, p = 0.749). However, satogaeri was associated with increased support from the mothers' families (65% vs. 55%, p = 0.013) and decreased support from neighbors (0% vs. 3%, p = 0.013). More mothers in the satogaeri group tended to lack knowledge of the city's childcare services (16% vs. 11%, p = 0.052). CONCLUSIONS: Satogaeri was not associated with the health status of mothers and infants at the 4-month checkup. The possible effects of satogaeri suggest the necessity for continuous support for families that select satogaeri.

    Japan’s rapidly aging population necessitates new approaches that enable citizens to actively participate in caring for themselves and others. However, a comprehensive framework defining the specific competencies needed for this critical community role has not yet been established. This study, therefore, aimed to define the novel concept of “care competency” and establish a consensus on its comprehensive component list for community citizens. We defined care competencies and developed a list using a modified Delphi technique (RAND/University of California, Los Angeles) involving 10 nursing researchers. Items were adapted from Japan’s Model Core Curriculum for Nursing Education, and a total of 528 items were evaluated and refined. In this study, care competency was defined as the complex ability to acquire and utilize knowledge and skills, based on evidence and intentional choices, to maintain the well-being of oneself, loved ones, and people in the community. The Delphi process identified 151 care competency items. This study thereby presents a novel framework that provides a foundation for developing globally applicable educational programs to foster mutual support and effective caregiving.

    AIM: To clarify how public health nurses allocate their time across various tasks and compare time-use patterns between prefectural and municipal public health nurses in Japan, where distinct administrative mandates define their roles. DESIGN: A self-reported, prospective time study. METHODS: Public health nurses from two municipalities and one prefecture recorded their daily practices and time allocations using Kintone. Time allocation differences by administrative level and job position were analysed using linear mixed-effects models. RESULTS: Overall, 121 participants contributed 9502 person-days of data. Participants spent an average of 463.6 min/workday on work-related activities. Municipal public health nurses dedicated more time to application paperwork (64.0 min), health examinations (57.6 min), and individual coordination (48.3 min). Prefectural public health nurses allocated more time to business management and organisational operations (69.0 min) and traveliing, particularly in rural contexts. Managers spent less time on direct care and more on administrative tasks. CONCLUSION: This study provides the first quantitative, self-reported evidence of task distribution among public health nurses across administrative levels and positions. The findings reflect structural differences in role expectations and underscore the need to reallocate workloads to better align with each level's mandate. IMPLICATIONS FOR THE PROFESSION: Task prioritisation, information and communication technology tool integration, and administrative support are essential in optimising public health nurses' contributions to community health. These strategies can reduce non-clinical workload and enable public health nurses to focus on high-impact, value-added public health services that promote health equity. IMPACT: This study closes a longstanding gap by quantifying PHNs' time allocation, revealing the hidden burden of administrative work, and providing critical insights for workforce planning and sustainable public health service delivery.     OBJECTIVES: This study demonstrated the usability of a co-designed benchmarking dashboard for supervisory public health nurses (PHNs) and their staff and identified supervisory PHNs' management challenges and strategies for system use. DESIGN: Descriptive case study. SAMPLE: Fifty-seven PHNs from three municipalities participated in the study using snowball sampling. MEASUREMENTS: Feedback was collected before, during, and after the dashboard development on the Kintone app. Participant attrition and System Usability Scale (SUS) scores were used to evaluate the dashboard's feasibility and usability using descriptive statistics. Content analysis was conducted to identify supervisory PHNs' management challenges and strategies for system use. RESULTS: There were no dropouts. The dashboard achieved an SUS score of 67.5, slightly higher than that of the existing system (66.0). Management challenges included human resource development, managing numerous individual cases, and conducting communityased activities. Dashboard use strategies included advancing evidence-based human resource development and strengthening data-driven management. CONCLUSIONS: Per the findings, strategies for developing the benchmarking dashboard supported evidence-based workforce management and promoted reflective practices, suggesting that dashboard systems can enhance supervisory PHNs' leadership and improve the quality and equity of community health services.     AIMS: To assess the reliability and validity of a negative emotion scale for public health nurses conducting child abuse prevention activities. DESIGN: A four-phase mixed method design. METHODS: Participants were public health nurses with experience in child abuse prevention across Japan. The initial scale was developed and refined based on previous studies, expert panel reviews, interviews with public health nurses, and a questionnaire-based pilot study. In 2024, 549 public health nurses completed a questionnaire on scale items. Item analysis, exploratory factor analysis, confirmatory factor analysis, comparisons with a preexisting scale, the number of times negative attitudes and the types of behaviours exhibited by parents were used to narrow the scale and assess its psychometric properties. RESULTS: Item analysis and exploratory factor analysis reduced the scale to 15 items on two factors: 'negative emotions toward aggressive and emotional attitudes' and 'negative emotions toward rejecting and uncooperative attitudes.' Confirmatory factor analysis indicated a good model fit. The Cronbach's alpha was high, and the negative emotions scale score positively correlated with the pre-existing scale, negative attitudes and parental behaviours. CONCLUSIONS: The Cronbach's alpha coefficient and other factors confirmed the scale's reliability, and correlations with other scales confirmed its validity. IMPLICATIONS FOR THE PROFESSION: Evaluating negative emotions provides critical insights into the quality of support and its influence on psychological well-being. IMPACT: By assessing negative emotions that public health nurses find difficult using this scale, the support system for them can be examined. REPORTING METHODS: STROBE checklist for cross-sectional studies was followed. PATIENT OR PUBLIC CONTRIBUTION: Public health nurses were involved in the generation of items for the scale. Their perspective was sought in determining the items for the scale. TRIAL AND PROTOCOL REGISTRATION: Registered in the UMIIN Clinical Trial Registry (UMIN-CTR ID UMIN000054650).

    BACKGROUND: Ultrasound hip screening is crucial for the early detection of developmental dysplasia (DDH), but little is known about standardized training approaches for nurses. This study aimed to evaluate an educational program combining web-based learning and objective structured clinical examination (OSCE) training to improve nurses' knowledge and skills in ultrasound hip screening and DDH prevention during universal neonatal and infant home visits. METHODS: The program targeted two objectives: (1) to enable nurses to understand the Graf method and acquire standard plane ultrasound images, and (2) to equip them to provide preventive health education on DDH. The intervention included five web-based modules and a two-day OSCE-based hands-on program, supervised by orthopedic pediatric surgeons. Outcomes included (1) knowledge test scores obtained from the web-based modules (range: 0-50 points), (2) OSCE scores (36 items, range: 0-360; global rating ≧3 on a 1-6), including delivery of preventive health education, (3) ability to capture a standard plane image using a phantom within three minutes, and (4) ability to capture a standard plane image with infant volunteers, assessed by medical supervisors. RESULTS: Nineteen participants (18 public health nurses and one midwife) from three municipalities completed the program. The mean knowledge test score was 47.2 (SD: 2.2), and the mean OSCE score was 329.3 (SD: 21.7), with a global rating of 4.4 (SD: 0.7). All participants conducted the preventive health education component. Eighteen participants (right hip) and 17 (left hip) successfully captured standard plane images using a phantom within three minutes. In scans of 39 infant volunteers, 34 scans (87.2%) successfully captured the standard plane; participants self-reported 31 as successful, though two were objectively classified as failures. CONCLUSIONS: The educational program improved nurses' ultrasound hip-screening skills and their capacity to deliver preventive health education during universal home visits. These findings support the potential feasibility of expanding nurse-led ultrasound screening and DDH prevention efforts in community settings.     ABSTRACT

    Objective

    This study aimed to develop essential individual care needs assessment (EICNA) items and evaluate the validity of that judgment.

    Design

    We used a sequential two‐phase design for this study.

    Methods

    Item selection was conducted using φ coefficients between these items’ values and the care need levels and discussions with supervisory PHNs. Phase 1 was a cross‐sectional, nationwide survey of 275 mid‐level public health nurses (PHNs) from 196 municipalities in Japan (December 2022 to January 2023), including 46 potential EICNA items. In Phase 2, PHNs piloted the EICNA items in clinical settings, entering data into a web‐based system that used an algorithm to determine care need levels based on the weighted sum of 21 items (August 2023 to January 2024). Thereafter, the PHNs evaluated the appropriateness of the algorithm's judgments.

    Results

    Twenty‐one essential items were identified. Among 1867 cases, care need levels were categorized as low (n = 1008, 54.0%), moderate (n = 652, 34.9%), and high (n = 207, 11.1%), with 94.9% of PHNs considered the algorithm's classifications appropriate.

    Conclusion

    Twenty‐one EICNA items were identified to assess the care needs, and the level of care needs determined by the weighted sum of these items was deemed appropriate by PHNs.

    Introduction

    Early identification of developmental dysplasia of the hip (DDH) is necessary to minimize its negative effects. Ultrasound screening is useful for detecting DDH in hospitals. Awareness about community-based screening systems is low in Japan. Despite established nationwide home visiting services and child health checkups in the country, more than 10% of DDH patients are diagnosed at the age of ≥1 year. This review aimed to clarify the status of universal ultrasound screening for DDH among infants in community settings in Japan.

    Methods

    The electronic databases of Igaku Chuo Zasshi, MEDLINE, CHINAL, ERIC, and APA PsycInfo were searched for articles published between 2002 and 2022. Articles were evaluated with the reach, effectiveness, adoption, implementation, and maintenance framework.

    Results

    In total, 148 articles were identified. Two articles were manually added, and 67 articles were excluded through abstract reviews, of which 20 were duplicates. Finally, 18 articles were included in the analysis. There are two types of universal ultrasound screening in community settings: municipality-led and hospital-led. Since 1992, municipality-led screening has been conducted during public infant health checkups in five municipalities. Six hospitals implemented ultrasound screening. The participation rate was around 90%. The Graf method is typically used for this purpose. The prevalence of abnormal hips was 3.6%–16.6%. Owing to limited human resources and skills in ultrasound, all studies mentioned the necessity of a universal screening system for the early detection of DDH.

    Conclusion

    Embedding universal ultrasound screening in community health checkup systems enables collaboration between healthcare professionals and caregivers to improve health inequities and ensure early detection of DDH cases.

    Ultrasound hip screening is suitable for the early identification of developmental dysplasia of the hip (DDH). Newborn and infant home visits are good opportunities for hip screening in the community, but studies focusing on nurse-led screenings are lacking. Based on a pre–post design, this study aims to develop and evaluate an ultrasound training program to improve nurses’ assessment skills in detecting DDH cases during newborn and infant home visits. Said educational program will include e-learning, hands-on seminars, and clinical training. The primary outcome will be the success rate of imaging standard planes (standardized images for hip assessment) in clinical training. The secondary outcomes will include knowledge test results, objective structured clinical examination scores, time required for imaging, and inter-rater reliability between nurses and physicians. The educational program will address the issue of missed and late detection of DDH cases in resource-limited communities. This study will demonstrate the feasibility of procedures and the effectiveness of the educational program in 2024. The protocol was registered in the University Hospital Medical Information Network Clinical Trial Registry before starting the study (no. UMIN000051929, 16 August 2023).

    OBJECTIVE: This study aimed to identify predictors of parenting difficulties at 18 months from the results of the 4-, 6-7-, and 9-10-month infant health checkups among caregivers who had not experienced difficulties at 4 months. DESIGN AND SAMPLES: This retrospective study used data from infant health checkups conducted in a city in Tokyo from November 2019 to October 2021. The participants were caregiver-child dyads of children who had undergone four checkups. Low birthweight, preterm, and multiple birth infants, and caregivers who experienced difficulties at 4 months were excluded. MEASUREMENTS: Data included caregiver, child-rearing environment, and child factors at the 4-, 6-7-, and 9-10-month checkups and caregivers' self-reported parenting difficulties at the 18-month checkup. RESULTS: Of the 555 caregivers, 48 (8.6%) experienced parenting difficulties at 18 months. Logistic regression analyses showed that mothers' physical condition (4 months), children being male, abnormal child growth (4 months), less than 10th percentile for children's height (6-7 and 9-10 months), and abnormal examination results (6-7 months) were significantly associated with parenting difficulties at 18 months. CONCLUSIONS: To prevent child abuse, public health nurses should consider the identified factors to detect and support caregivers with emerging parenting difficulties.

    Early detection of developmental dysplasia of the hip (DDH) in children is crucial. Due to COVID-19, maternal and child health services have been suspended temporarily, increasing the risk of late detection of DDH. This study aimed to reveal Japan’s current situation regarding community hip screening for newborns and infants and to provide health guidance for caregivers regarding DDH. A web-based, nationwide cross-sectional survey was conducted between February and March 2023 (n = 1737). One public health nurse overseeing maternal and child health per municipality responded to the 2022 municipality hip screening system. Among the 436 municipalities that responded (response rate: 25.1%), 97.5% implemented hip screening within 4 months, and approximately 60% performed it during newborn home visits, while only 2.3% conducted hip ultrasound screening. Perfect checking of the risk factors for DDH during newborn home visits and training opportunities for home visitors must be improved. Educational programs regarding DDH for home visitors and caregivers are needed to prevent the late diagnosis of DDH. Furthermore, collaboration between pediatric orthopedic surgeons and nurses is crucial for developing effective community-based hip-screening systems by bridging the evidence and practice gap in the early detection of DDH.

    Background

    Digitalization and information and communication technology (ICT) promote effective, efficient individual and community care. Clinical terminology or taxonomy and its framework visualize individual patients’ and nursing interventions’ classifications to improve their outcomes and care quality. Public health nurses (PHNs) provide lifelong individual care and community-based activities while developing projects to promote community health. The linkage between these practices and clinical assessment remains tacit. Owing to Japan’s lagging digitalization, supervisory PHNs face difficulties in monitoring each department’s activities and staff members’ performances and competencies. Randomly selected prefectural or municipal PHNs collect data on daily activities and required hours every 3 years. No study has adopted these data for public health nursing care management. PHNs need ICTs to manage their work and improve care quality; it may help identify health needs and suggest best public health nursing practices.

    Objective

    We aim to develop and validate an electronic recording and management system for evaluating different public health nursing practice needs, including individual care, community-based activities, and project development, and for determining their best practices.

    Methods

    We used a 2-phase exploratory sequential design (in Japan) comprising 2 phases. In phase 1, we developed the system’s architectural framework and a hypothetical algorithm to determine the need for practice review through a literature review and a panel discussion. We designed a cloud-based practice recording system, including a daily record system and a termly review system. The panels included 3 supervisors who were prior PHNs at the prefectural or municipal government, and 1 was the executive director of the Japanese Nursing Association. The panels agreed that the draft architectural framework and hypothetical algorithm were reasonable. The system was not linked to electronic nursing records to protect patient privacy. Phase 2 validated each item through interviews with supervisory PHNs using a web-based meeting system. A nationwide survey was distributed to supervisory and midcareer PHNs across local governments.

    Results

    This study was funded in March 2022 and approved by all ethics review boards from July to September and November 2022. Data collection was completed in January 2023. Five PHNs participated in the interviews. In the nationwide survey, responses were obtained from 177 local governments of supervisory PHNs and 196 midcareer ones.

    Conclusions

    This study will reveal PHNs’ tacit knowledge about their practices, assess needs for different approaches, and determine best practices. Additionally, this study will promote ICT-based practices in public health nursing. The system will enable PHNs to record their daily activities and share them with their supervisors to reflect on and improve their performance, and the quality of care to promote health equity in community settings. The system will support supervisory PHNs in creating performance benchmarks for their staff and departments to promote evidence-based human resource development and management.

    Most nursing simulation programs focus on persons’ healthcare needs in hospital settings, and little is known about how to identify them in home settings. This study aims to develop and validate a virtual reality (VR) simulation program for nursing students to improve their clinical reasoning skills and confidence in assessing persons’ healthcare needs in home settings. We developed a VR simulation program based on a literature review and expert discussion. In Phase 1, home visit nurses or public health nurses will validate the program through their interviews in 2022. In Phase 2, we will conduct a pilot and main single-blinded randomized trial for nursing students to confirm the effectiveness from 2022 and 2023. Participants will be randomly allocated into an intervention group using VR simulations and a control group receiving videos regarding three kinds of community residents’ lives [1:1]. After obtaining informed consent, the students will submit their anonymous data to the researchers to prevent associating their grade evaluation. The primary outcome will be their clinical reasoning skills. The second outcome will include their satisfaction and self-confidence. This study will examine the effectiveness of improving their clinical reasoning skills and confidence in assessing persons’ healthcare needs in home settings.

    Objective This study aimed to explore the support extended by prefectural public health nurses (PHNs) toward the human resource development (HRD) of municipal PHNs in Japan.Methods We performed a qualitative descriptive study involving nine prefectural PHNs from April 2019 to May 2020. The data were collected through semi-structured interviews using an interview guide, described qualitatively, coded, and then categorized.Results Five categories were extracted. "Clarifying the needs and problems related to HRD and daily PHN activities in the municipalities," "Creating an environment where all municipal PHNs have equal opportunities for off-the-job training," and "Helping municipal PHNs recognize the meaning of practice and develop an evaluation perspective" were extracted from the prefectural government PHNs and prefectural health center (HC) PHNs. "Clarifying problems and future prospects to encourage the growth of PHNs" and "Creating an environment where the significance and value of the activities of PHNs are recognized within the organization and HRD can easily take place" were extracted from the HC PHNs.Conclusion Much of the HRD support provided by the prefectural PHNs to the municipal PHNs was analogous to the PHN activities provided to the community and residents. To promote HRD effectively, prefectural PHNs should apply their individual care skills to the HRD of municipal PHNs.

    This cross-sectional study aimed to (1) describe the unclassified contents of telephone consultation services provided by a public health center during the first wave of COVID-19 in Japan and (2) examine whether the contents required assistance from public health nurses (PHNs). We analyzed a total of 207 calls in which the purpose of the call was unclassified into pre-set categories. PHNs transcribed the exact text of the consultation conversations recorded from 25 March to 20 April 2020 in City A. Approximately half of the calls were from residents. Seven categories were extracted through a qualitative content analysis. The most common topic was infection control measures, where the presence of COVID-19 infection was assumed (n = 62); the second most common was extreme anxiety and fear of infection (n = 50). Questions about the COVID-19 response system (n = 30), discrimination and misunderstandings about COVID-19 (n = 24), and response measures for COVID-19 outbreaks within organizations (n = 18) were also included. The unclassified consultations included various topics, several of which required the expertise of a PHN. Each local government should consider sharing and task-shifting telephone consultation services among PHNs and other staff to reduce their burden and allow them to concentrate on conducting infection control more effectively.

    This study aimed to (1) describe the characteristics of community residents who used coronavirus disease 2019 (COVID-19)-related telephone consultation systems by public health nurses (PHNs) and (2) analyze the concerns they had during the first wave of COVID-19 in Japan. Among 1126 telephone consultations, PHNs recorded telephone consultations between 25 March, and 30 April, 2020, in City A, Japan. We analyzed 1017 consultations involving 799 (79%) community residents (resident group) and 218 (21%) organizational representatives (organization group) located in City A. Half of the consultations were made during midmorning, and most of the consultations were regarding COVID-19 symptoms. Among the resident group, visiting a primary care doctor was the most common recommendation by the PHNs; there was no difference in provision of consultation by sex. Health- and welfare-related organizations mainly consulted PHNs about "having COVID-19-related symptoms" and "undergoing PCR testing," and PHNs' recommended them to visit a primary care doctor and coordinate PCR testing. The results suggest that public health centers should provide more helpful information on COVID-19 that accurately reflects the concerns of the population.

    The number of coronavirus disease 2019-infected people taking treatment at home is increasing every day in Japan. Even though they need to be hospitalized, due to the inability to secure childcare, infected parents are taking medical treatment at home. This short report focused on developing a new childcare system in Japan for parents infected with coronavirus disease 2019. It is important that each public sector makes family-friendly policies to ensure the development of a childcare system equipped to deal with the current pandemic.

    A combination of natural calamities and coronavirus disease 2019 (COVID-19) would cause a complex emergency. Modifying a disaster prevention plan to combat COVID-19, by developing an evacuation and monitoring system for the COVID-19 cases and their contacts who stay at their homes, is urgently needed in the public sector. This short communication presents a preparation for the complex disaster by public health nurses who could contribute to preventing risks and death of community-dwelling people.

    COVID-19の第一波感染拡大時に保健所が直面していた課題を、住民や医療機関からの電話相談記録、保健師への質問紙調査やインタビューによって明らかにした。電話相談記録の分析から、住民のみならず医療機関や企業等から相談があったこと、保健師等の専門職が対応すべき内容とタスクシフトできる内容があったこと等が示された。ケーススタディーの結果からは急激な感染拡大に伴う業務の増大、組織内の混乱と疲弊、住民サービスの縮小から変更・修正による復活までのプロセスが明らかになった。今後の感染拡大や未知の感染症発生に対応可能な地域保健医療システムの構築について提言した。

    3歳時健診対象者633人に対し質問紙を郵送し、健診を受診した(対象児の保護者)550人中、質問紙に回答した369人から回答に欠損のあった31人を除いた336人を対象に、家の中で起こる子どもの不慮の事故による傷害の関連要因について検討した。転落、誤飲、やけど、おぼれの四つについては受診の有無を問わずいずれかの事故にあった子どもは全体の6割で、受診を伴う事故の経験者は13%、受診を伴わない事故の経験者は48%であった。受診の有無に関わらず各事故を経験した割合は転落46.4%、誤飲9.9%、やけど15.3%、おぼれ11.3%であった。家庭内で予防策を実施していない親については、子どもの基本的生活習慣や家族の習慣の実施状況との関連がみられ、健診で把握し予防策の実施に向けて介入できる可能性が考えられた。

    本研究の目的は、アセスメント対象者の「家での暮らし」に関する看護学生の臨床推論力を高めるため、Virtual reality(以下、VRとする。)環境を用いた協調学習を円滑に行う遠隔シミュレーション教育プログラムを開発することである。初年度は、協調学習の方法論を活用したVR遠隔シミュレーション教育プログラムとシステムを構築することを目的として、研究班会議を合計7回開催した。学生が (1) 家の中の様子から対象者の状態を推定し、(2)疾病や健康課題を予測すると共に、 観察すべき家の中のポイントを理解し素早く(1)(2)を実行の3段階の能力を獲得できるようにVR教材を開発した。なお、対象者は世帯人数と観察ポイントを拡充させるために独居、夫婦2人暮らし、3人家族を設定した。また、VR環境内で体験者である学生が自身の家庭訪問の様子をネット越しに教員やグループ学生と共有し、音声・視点のやり取りを同期的に行えるシステムとして、Zoomを活用する方法論について検討した。現在、VR教材を分かりやすいものとするために、色調や映写時間の修正を行っているところである。また、2年目に実施するパイロット調査に向けて、評価項目の選定、尺度の日本語訳の作成を行っているところである。学生の自己評価の尺度に関しては、古い尺度は作成者の使用許諾請求が困難であったため、使用許諾不要のNational League for Nursingの尺度を日本語に翻訳して使用する予定である。

    新型コロナウイルス感染症のパンデミックにより、各自治体での住民サービスに影響が出ている中、妊娠~乳幼児の育児期の保護者への教育機会の場である母親・両親学級、育児グループ、乳幼児健康診査などの一次予防サービスの提供・利用が縮小を余儀なくされた。2021(令和3)年度は、それらの教育機会の減少により、保護者が傷害予防の情報を得られていない可能性を考え、家の中での乳幼児の傷害と事故リスクに関する状況、傷害予防の情報を得る機会の変化、情報提供方法の希望についてなど、地域の妊婦および母親の傷害予防教育の実態とニーズを把握することを目指した。
    方法としては、国内外の学会発表・調査報告・論文等からコロナ禍における育児に関する課題を収集・整理し、かつ乳幼児を育児中の母親・妊婦へのヒアリングを加味し質問項目を作成した。オンラインによるウェブ調査を実施し、妊婦および0-2歳児を育児中の母親750名から回答を得た。現在、データ分析中である。

    研究初年度である本年は、2017-2018年度に実施した比較試験の対象者X区131名に対し、第1子が2歳時点での、自記式質問紙郵送による追跡調査を開始した。
    【目的】妊娠期の事故予防集団教育プログラムの長期的効果の検証・評価
    【調査項目】ロジックモデルの評価項目に沿い、①事故の有無(病院受診あり/なし、事故の種類、通院・入院の有無)②現在の予防行動の実施状況③事故予防について人と話したことがあるか(積極的関与度)について測定
    【現状】2020年9月に郵送調査終了予定。収集したデータについて解析を行い、介入群・対照群を比較する。結果について分析後、対象者へのインタビュー調査結果と合わせて、プログラムのプロセス評価・アウトカム評価を行い、妊娠期の教育プログラム開発に反映させる予定である。
