    A limitation of widely used intraoral scanners (IOSs) is their inability to capture finish lines at the subgingival marginal area, as they only extract surface information. Swept-source optical coherence tomography (SS-OCT) captures high-speed, high-resolution cross-sectional images of soft and hard tissues. Integrating this technology can overcome clinical IOS limitations. Therefore, this study was conducted to fabricate crowns from three-dimensional images scanned with SS-OCT as a proof-of-principle for its application in IOSs and to evaluate fit accuracy. TRIOS3 was used for comparison, with both SS-OCT and TRIOS3 scanned three times, and crowns were fabricated using the same digital workflow. Internal gaps were measured using scanning electron microscopy, and marginal fit was evaluated via microscopy. Results showed that TRIOS3 had superior accuracy. SS-OCT can image solely in the occlusal direction, with accuracy decreasing at greater depths, which reduces precision around the margin. Additionally, SS-OCT lacks automatic correction of surface information in computer-aided design (CAD) software. To improve SS-OCT accuracy for abutment tooth measurements, automatic margin correction, improved CAD compatibility, and specialized probes for capturing tooth features are needed.

    ABSTRACT

    Background

    Implant‐supported prostheses frequently face both mechanical and biological challenges. Although various techniques and principles are employed to manage excessive loads, occlusal contacts often change within 6 months of placement.

    Objective

    This review aims to analyse relevant studies to evaluate the changes in occlusal contacts that occur following the placement of superstructures in implant‐supported prostheses.

    Methods

    The protocol was designed according to the Preferred Reporting Items for Systematic Reviews and Meta‐Analyses Protocols (PRISMA‐P) and registered with the International Prospective Register of Systematic Reviews. Studies were included if their outcomes addressed stress distribution, occlusal force, occlusal contact, occlusal scheme or changes in occlusal time. Non‐English articles, case reports, animal experiments, systematic reviews and literature reviews were excluded.

    Results

    A total of 1867 articles were retrieved through database searches, along with an additional 43 articles identified through manual searches, of which 416 were duplicates. After screening the titles and abstracts, 1367 articles were excluded, and 12 were deemed non‐retrievable. The full texts of the remaining 85 articles were independently reviewed by two authors for eligibility, resulting in the exclusion of 77 articles based on the inclusion criteria. Ultimately, eight articles were included in the review.

    Conclusion

    Implant‐supported crowns gradually approach or surpass the load of natural teeth over time, as evidenced by increased occlusal force, larger contact areas and longer occlusion durations. These findings highlight the need for more accurate and advanced measurement techniques to better understand and manage the long‐term changes in occlusion associated with implant‐supported crowns.

    Abstract

    Aim:

    We aimed to evaluate the diagnostic performance of the You Only Look Once version 8 (YOLOv8) deep learning model in the automated detection of periodontal bone loss (PBL) on panoramic radiographs and assess its potential as a clinical diagnostic aid.

    Materials and Methods:

    A total of 500 annotated panoramic radiographs with PBL were retrospectively collected and randomized into training (n = 400), validation (n = 50), and testing (n = 50) datasets. Image annotation was performed using Roboflow and validated by a radiologist and periodontist. YOLOv8 models of five variants (n, s, m, l, and x) were trained using Google Colab. Model performance was evaluated via confusion matrix-derived metrics: accuracy, precision, recall, specificity, F1-score, and average detection time. False cases were also analyzed.

    Results:

    After training and optimization, YOLOv8-s showed the highest mAP result (83.7 ± 1.74%) compared to other variants. During testing, YOLOv8-s demonstrated the highest balance between speed and performance, achieving an accuracy of 90.27% (CI 95% [88.49, 92.05]), precision of 91.27% (CI 95% [89.25, 93.29]), recall of 94.64% (CI 95% [92.99,96.29]), specificity of 80.94% (CI 95% [76.76,85.12]), and an F1-score of 92.98% (CI 95% [91.22, 94.78]) at a 10% confidence threshold. The average detection time per image was 22.6 ms. The most frequent false positives were healthy teeth, while missed detections were mainly mild cases of PBL.

    Conclusion:

    The YOLOv8 model effectively detects PBL on panoramic radiographs with high accuracy and speed. It holds significant potential as an assistive tool in clinical diagnosis of periodontitis. Future research should focus on severity classification and multi-center validation to enhance clinical integration.

    Abstract

    The aim of this study was to investigate the bactericidal effect and recovery of biocompatibility of contaminated titanium surfaces using a combination treatment involving silver, copper, or iron ion application along with 400 nm purple-LED light irradiation. Additionally, the study sought to develop a functional calcium phosphate (CaP) coating treatment on titanium surfaces following disinfection, to promote re-osseointegration. A purple-LED emitting light at 400 nm was utilized to irradiate Staphylococcus aureus suspensions and biofilms in the presence of various concentrations of silver, copper, and iron solutions for 1 min. The bactericidal effect and electron spin resonance (ESR) spectrum were subsequently evaluated. Additionally, the hydrophilicity of the titanium surface and cell viability of MC3T3-E1 cells after combination treatment with silver ion was evaluated. Furthermore, a titanium surface coating with CaP gene transfection carrier containing plasmid DNA was developed using an electric current. The activity of hard tissue formation was then evaluated both in vitro and in vivo post-treatment. The bactericidal effect of the combination treatment with silver ions was attributed to the generation of hydroxyl radicals, whereas the effects from iron and copper treatments were not radical-mediated. The silver treatment significantly restored the hydrophilicity and cell affinity of the titanium surface. Moreover, CaP coating applied via an electric current (30 µA for 5 min) enhanced hard tissue formation activity on the titanium surface in both in vitro and in vivo settings. The combination treatment utilizing silver ions and purple-LED irradiation significantly enhanced bactericidal effects by generating high levels of hydroxyl radicals. Additionally, coating the titanium surface with functionalized CaP promoted early osseointegration, suggesting a promising strategy for improving implant outcomes.

    Objectives: This study aimed to evaluate the effectiveness of fully remote practical training for the preparation of removable partial dentures, using the video meeting platform Zoom and the communication app. LINE. Materials and Methods: Remote training was applied for one student using Zoom and LINE at the same time as in-person training for the rest of the class. Consumable and instruments needed were delivered to the student subject to remote training in advance. The student’s work was assessed in front of a web camera, at a pre-determined checkpoint during each step. The actual appliances were then mailed to the instructors and reevaluated. The extent of agreement among the assessors for the video-based and direct evaluations, as well as the extent of agreement between the video-based and direct evaluations were calculated. Results: The introduction of Zoom facilitated mutual communication between the student and the instructors. LINE has proven beneficial for practical training. A fully remote practical training class can be conducted smoothly by assigning a dedicated instructor for remote training. The Κ coefficient for agreement between monitor-based and direct evaluations was 0.704 ± 0.12. The survey results indicated that this practical training style was effective in enhancing knowledge and skills. Conclusions: Fully remote blended learning practical training is feasible using Zoom and LINE; however, the tasks to be conducted must be carefully selected. This study is a case report of a single student; thus, future studies should involve fully remote training of small groups to evaluate its educational effectiveness.

    血管柄付き遊離腓骨皮弁による下顎骨再建治療を受けた患者1名の臨床データを用いて、術後のフォローアップCT画像解析による再建下顎骨の骨接合部のリモデリング解析、CTデータベースの有限要素解析による再建下顎骨のメカニカルストレス分布解析、これらの解析をもとにした患者固有の骨リモデリングアルゴリズムの構築とその応用を試みた。下顎右側臼歯部の歯肉扁平上皮癌のため下顎骨区域切除術ならびに血管柄付き遊離腓骨皮弁による下顎骨再建術を受けた66歳男性の治療データを用いて一連の解析を行った。下顎骨再建手術直後にCT撮影を実施し、その後は4、16、28ヵ月後に術後フォローアップとしてCT撮影を実施した。オトガイ部の下顎骨-腓骨の接合部をDS1、1ヶ所の腓骨同士の接合部をDS2、顎関節部に近い下顎骨-腓骨の接合部をDS3と定義した。その結果、特に異種骨(下顎骨-腓骨)間接合であるDS1、DS3において骨形態の経時的変化は顕著であり、各接合部において歪みエネルギー密度分布は経時的に大きく変化していた。構築したアルゴリズムを用いたシミュレーション解析により、下顎骨-腓骨間に適切な骨接合が獲得されることで、咬合力が増大しても固定用チタンプレートの破折リスクは増加しないことが示された。

    Mandibular deviation (MD) is a common reconstruction sequela after segmental mandibulectomy. Although proper postoperative rehabilitation is critical for MD management and minimization, the information available is limited. This report describes postoperative rehabilitation with an occlusal splint fabricated using computer-aided design (CAD) and computer-aided manufacturing (CAM) (CAD/CAM-OS) and the results of a threedimensional occlusal analysis using an intraoral scanner after hemimandibulectomy and plate reconstruction. Despite the short follow-up, adherence to postoperative rehabilitation with CAD/CAM-OS for MD correction, even during radiotherapy, was demonstrated by the digital workflow and analysis results.

    The removal of subgingival deposits, especially calculus, plays a crucial role in basic periodontal therapy. However, manual detection methods affect accuracy owing to the operator's skill. To avoid this uncertainty, we have developed a calculus detection device named "Sensor probe" and evaluated its ability to detect calculus for future clinical applications. The Sensor probe consisted of a 635 nm-wavelength semiconductor laser and a 0.5 mm-diameter single-mode optical fiber. Initially, the performance of the device was evaluated using clinically obtained extracted teeth with calculus covered with a stainless-steel shielding plate with pinhole. Then, the effect of the optical fiber's end shape on calculus detection performance was analyzed. Lastly, the performance of the Sensor probe was compared to that of a conventional periodontal probe in terms of accuracy, sensitivity, and specificity for calculus detection using calculus-covered extracted teeth. The results indicated that Sensor probe detected dental calculus through the pinhole with a diameter of 300 μm or more when applied from a distance of 100 μm. The results analyzing the effect of the optical fiber's end shape on calculus detection performance showed that cutting the fiber end at an angle of 45° resulted in the most effective calculus detection. This may be because the laser light refracted on the cut surface and concentrated on the fiber side. Moreover, by comparing the performance of this device to a conventional periodontal probe revealed that the Sensor probe showed improved calculus detection accuracy in deeper periodontal pockets. This improvement was particularly significant in the apical third, where detection is typically difficult. In conclusion, a Sensor probe that uses an optical fiber with a 45° angled end may facilitate subgingival calculus detection. In future clinical applications, Sensor probes could lead to more accurate and efficient calculus removal, especially for deeper periodontal pockets.

    BACKGROUND: The masseter muscle has a complicated multipennate internal structure and exhibits functional differentiation when performing various stomatognathic functions. It is important to understand the internal structural changes of the muscle during functioning to elucidate characteristic muscle disorders such as local myalgia. Diffusion tensor imaging (DTI) may be useful for investigating the internal structural features of muscle. OBJECTIVES: To evaluate the features of masseter muscle fibres in human participants using DTI fibre tractography, and to elucidate the structural differences in the masseter muscle between the mandibular rest and open positions. METHODS: Five healthy men (age 31 ± 7 years) underwent DTI and T1-weighted MRI of the right masseter muscle in the mandibular rest and open positions. MR images were used as a reference for muscle layer segmentation (superficial, intermediate, and deep). DTI fibre tractography of the masseter muscle was performed and the orientation of the DTI fibres was analysed in each layer using coordinates based on the Frankfurt horizontal plane. RESULTS: The DTI fibre orientation of the deep layer significantly changed between the mandibular rest and open positions in the frontal plane (p < 0.05, Wilcoxon rank sum test). However, no significant change was found in the superficial and intermediate layers. CONCLUSION: DTI fibre tractography confirmed regional differences in the orientation change of the masseter muscle fibres between different mandibular positions. The results may support the existence of functional partitioning inside the masseter muscle and suggest that DTI may be useful for the evaluation of muscle fibres in multipennate muscles.

    The purpose of this study was to investigate the effect of gas species used for low-temperature atmospheric pressure plasma surface treatment, using various gas species and different treatment times, on zirconia surface state and the bond strength between zirconia and dental resin cement. Three groups of zirconia specimens with different surface treatments were prepared as follows: untreated group, alumina sandblasting treatment group, and plasma treatment group. Nitrogen (N2), carbon dioxide (CO2), oxygen (O2), argon (Ar), and air were employed for plasma irradiation. The bond strength between each zirconia specimen and resin cement was compared using a tension test. The effect of the gas species for plasma irradiation on the zirconia surface was investigated using a contact angle meter, an optical interferometer, an X-ray diffractometer, and X-ray photoelectric spectroscopy. Plasma irradiation increased the wettability and decreased the carbon contamination on the zirconia surface, whereas it did not affect the surface topography and crystalline phase. The bond strength varied depending on the gas species and irradiation time. Plasma treatment with N2 gas significantly increased bond strength compared to the untreated group and showed a high bond strength equivalent to that of the sandblasting treatment group. The removal of carbon contamination from the zirconia surface and an increase in the percentage of Zr-O2 on the zirconia surface by plasma irradiation might increase bond strength.

    The Tohoku University Hospital has been a clinical and research facility for all the related departments of Tohoku University. Medical-dental and interprofessional collaboration has resulted in special treatment teams, made up of members of departments such as the center for head and neck cancer, the center for dysphagia, and the cleft lip and palate center. Those treatment teams held conferences, case study meetings, reading sessions, and in-hospital seminars. The purpose of this study was to evaluate the outcomes of various medical-dental and the interprofessional collaboration at Tohoku University Hospital and training program to equip hospital dentists in higher medical institutions. The attainment targets are the acquisition of basic medical skills and knowledge under the guidance of supervising doctors. As a result, the hospital dentists could acquire their own specialized knowledge and skills certificated by each academic society. The smooth team treatment has been achieved, and the number of cases discussed by cancer boards and center for dysphagia has increased year by year due to the efficiency of their clinical pathways. On the dental care side as well, the wearing rates of maxillofacial prosthetic devices such as maxillofacial prostheses and palatal augmentation prostheses (PAP) have improved, which have contributed to improving patient's stomatognathic function. Tohoku University Hospital has been practicing collaboration between medical and dental professionals and it has produced mutual benefits. Our interprofessional training system based on the medical-dental collaboration could develop professionals who have acquired cross-disciplinary knowledge and skills from experienced doctors.

    In the last few years, artificial intelligence (AI) research has been rapidly developing and emerging in the field of dental and maxillofacial radiology. Dental radiography, which is commonly used in daily practices, provides an incredibly rich resource for AI development and attracted many researchers to develop its application for various purposes. This study reviewed the applicability of AI for dental radiography from the current studies. Online searches on PubMed and IEEE Xplore databases, up to December 2020, and subsequent manual searches were performed. Then, we categorized the application of AI according to similarity of the following purposes: diagnosis of dental caries, periapical pathologies, and periodontal bone loss; cyst and tumor classification; cephalometric analysis; screening of osteoporosis; tooth recognition and forensic odontology; dental implant system recognition; and image quality enhancement. Current development of AI methodology in each aforementioned application were subsequently discussed. Although most of the reviewed studies demonstrated a great potential of AI application for dental radiography, further development is still needed before implementation in clinical routine due to several challenges and limitations, such as lack of datasets size justification and unstandardized reporting format. Considering the current limitations and challenges, future AI research in dental radiography should follow standardized reporting formats in order to align the research designs and enhance the impact of AI development globally.

    PURPOSE: To evaluate the effect of different procedures for retightening the abutment screw on the screw stability between the abutment and implant. MATERIALS AND METHODS: External hexagon connection (EHC) and internal tri-channel connection (ITC) implants were employed in this study. Each abutment screw was used for the following applications (n = 5 for each group): group 0 = abutment was tightened and retightened with a 10-minute interval; group 1 = abutment was tightened and retightened with a 10-minute interval, then loaded for 500,000 cycles; group 2 = abutment was loaded for 83,000 cycles during tightening and retightening and then loaded for 500,000 cycles; group 3 = abutment was loaded for 167,000 cycles during tightening and retightening and then loaded for 500,000 cycles; and group 4 = abutment was loaded for 250,000 cycles during tightening and retightening and then loaded for 500,000 cycles. Both tightening and retightening torques were set at 35 Ncm, and dynamic oblique loading between 0 N and 200 N was applied on the abutment in all groups. The settlement of the abutments (settling value) after retightening and the abutments' removal torque values (RTVs) after testing were measured. RESULTS: No significant differences in settling values were found for EHC or ITC. There were significant differences in the RTVs on EHC implants, but ITC implants showed no difference in RTV with different prosthetic retightening applications. CONCLUSION: The retightening application affected the joint stability of EHC implants, but did not affect the settlement or joint stability of ITC implants.

    BACKGROUND: While hand and ultrasonic scalers are the primary tools used for the removal of dental calculus in periodontal treatment, many studies have shown that they also damage the enamel surface. We have developed a novel actuator-driven pulsed water jet (ADPJ) system, which has the ability to selectively remove materials depending on their stiffness. Considering the different material properties between teeth and dental calculus, it might be possible to develop the ADPJ to remove dental calculus without damage to the tooth's enamel surface using a suitable jet pressure. Therefore, the aim of this study was to assess the effectiveness of the ADPJ in removing dental calculus, and the surface features of the teeth after its use. METHODS: A total of 93 artificial teeth coated with artificial dental calculus were examined in this study. The weights of 90 teeth were measured before and after the use of ADPJ, which had an applied voltage setting of 150, 200, or 240 V. The three remaining teeth were instrumented with a conventional hand scaler, ultrasonic scaler, or ADPJ (set at 240 V). Damage to the artificial tooth surfaces was evaluated using 5% Evans blue dye under an optical microscope. Furthermore, apatite pellets, which are utilized as experimental substitutes for natural teeth, were assessed after the use of ADPJ and both conventional scalers. RESULTS: The ADPJ significantly reduced the amount of artificial calculus, and the removal rate was dependent on the applied voltage. No damage was observed on the surface of the artificial tooth and apatite pellet following the use of ADPJ, in contrast to the conventional scalers. CONCLUSIONS: The results of this study demonstrate the in vitro effectiveness of ADPJ in the removal of dental calculus, without causing damage to tooth surfaces.

    <title>Abstract</title>
    <sec>
    <title>Background</title>
    The impact of the jaw bone condition, such as bone quantity and quality in the implant placement site, affecting the accuracy of implant placement with computer-guided surgery (CGS) remains unclear. Therefore, this study aimed to evaluate the influence of bone condition, i.e., bone density, bone width, and cortical bone thickness at the crestal bone on the accuracy of implant placement with CGS.

    <sec>
    <title>Methods</title>
    A total of 47 tissue-level implants from 25 patients placed in the posterior mandibular area were studied. Implant placement position was planned on the simulation software, Simplant® Pro 16, by superimposing preoperative computed tomography images with stereolithography data of diagnostic wax-up on the dental cast. Implant placement surgery was performed using the surgical guide plate to reflect the planned implant position. The post-surgical dental cast was scanned to determine the position of the placed implant. Linear and vertical deviations between planned and placed implants were calculated. Deviations at both platform and apical of the implant were measured in the bucco-lingual and mesio-distal directions. Intra- and inter-observer variabilities were calculated to ensure measurement reliability. Multiple linear regression analysis was employed to investigate the effect of the bone condition, such as density, width, and cortical bone thickness at the implant site area, on the accuracy of implant placement (<italic>α</italic> = 0.05).

    <sec>
    <title>Result</title>
    Intra- and inter-observer variabilities of these measurements showed excellent agreement (intra class correlation coefficient ± 0.90). Bone condition significantly influenced the accuracy of implant placement using CGS (<italic>p &lt; 0.05</italic>). Both bone density and width were found to be significant predictors.

    <title>Conclusions</title>
    Low bone density and/or narrow bucco-lingual width near the alveolar bone crest in the implant placement site might be a risk factor influencing the accuracy of implant placement with CGS.

    BACKGROUND: Research on resolving implant fracture is still gaining attention as it can be a serious treatment failure outcome. The implant fracture is likely to occur due to increased stress in implant body associated with peri-implant bone resorption. OBJECTIVE: This study aimed to investigate the relationship between degree of peri-implant bone resorption and stress distribution in implant body by using finite element analysis (FEA). METHODS: The magnitude and direction of loads on implants at the mandibular molar region were measured with 3D piezoelectric force transducers in a patient during maximal voluntary clenching, grinding, and tapping to obtain input for the FEA as the loading condition. Simplified finite element bone models were generated to simulate the six patterns of peri-implant bone resorption progression which the bone levels from implant platform were set at 0, 1, 2, 3, 4, and 5 mm. Three types of implants with different diameters (3.5, 4.0, and 5.0 mm), corresponding abutments and screws were created. FEA using the in vivo measured load was conducted to investigate how peri-implant bone loss affects stress distribution in the implant body. RESULTS: Loading condition affected stress distribution in the implant body. As bone resorption increased, von Mises stress in the implant body became larger, which was marked in smaller diameter implants, and this change was amply demonstrated when the bone resorption increased from 0 to 2 mm. CONCLUSION: The FEA demonstrated that as peri-implant bone loss increased, the magnitude of stress in the implant increased, especially in small diameter implants.

    We developed a calcium phosphate-based paste containing siRNA against TNF-α and investigated its anti-inflammatory and bone-healing effects in vitro and in vivo in a rat periodontitis model. The bioactive spherical CaP/PEI/siRNA/SiO2 nanoparticles had a core diameter of 40-90 nm and a positive charge (+23 mV) that facilitated cellular uptake. The TNF- α gene silencing efficiency of the nanoparticles in J774.2 monocytes, gingival-derived cells, and bone marrow-derived cells was 12 ± 2%, 36 ± 8%, and 35 ± 22%, respectively. CaP/PEI/siRNA/SiO2 nanoparticles cancelled the suppression of alkaline phosphatase (ALP) activity in LPS-stimulated bone marrow-derived cells. In vivo, ALP mRNA was up-regulated, TNF-α mRNA was down-regulated, and the amount of released TNF-α was significantly reduced after topical application of the calcium phosphate-based paste containing siRNA-loaded nanoparticles. The number of TNF-α-positive cells in response to CaP/PEI/siRNA/SiO2 nanoparticle application was lower than that observed in the absence of siRNA. Elevated ALP activity and numerous TRAP-positive cells (osteoclasts) were observed in response to the application of all calcium phosphate pastes. These results demonstrate that local application of a paste consisting of siRNA-loaded calcium phosphate nanoparticles successfully induces TNF-α silencing in vitro and in vivo and removes the suppression of ALP activity stimulated by inflammation. STATEMENT OF SIGNIFICANCE: We developed a calcium phosphate-based paste containing nanoparticles loaded with siRNA against TNF-α. The nanoparticles had a core diameter of 40-90 nm and positive charge (+23 mV). The anti-inflammatory and osteoinductive effects of the paste were investigated in vitro and in vivo in a rat periodontitis model. In vitro, the TNF-α gene silencing efficiency of the nanoparticles in J774.2 monocytes, gingival-derived cells, and bone marrow-derived cells was 12 ± 2%, 36 ± 8%, and 35 ± 22%, respectively. The ALP activity of bone marrow-derived cells was recovered. In vivo, TNF-α mRNA was down-regulated and the amount of released TNF-α was significantly reduced, whereas the ALP mRNA was up-regulated. Elevated ALP activity and TRAP-positive cells were observed by immunohistochemistry.

    The human masticatory system has received significant attention in the areas of biomechanics due to its sophisticated co-activation of a group of masticatory muscles which contribute to the fundamental oral functions. However, determination of each muscular force remains fairly challenging in vivo; the conventional data available may be inapplicable to patients who experience major oral interventions such as maxillofacial reconstruction, in which the resultant unsymmetrical anatomical structure invokes a more complex stomatognathic functioning system. Therefore, this study aimed to (1) establish an inverse identification procedure by incorporating the sequential Kriging optimization (SKO) algorithm, coupled with the patient-specific finite element analysis (FEA) in silico and occlusal force measurements at different time points over a course of rehabilitation in vivo; and (2) evaluate muscular functionality for a patient with mandibular reconstruction using a fibula free flap (FFF) procedure. The results from this study proved the hypothesis that the proposed method is of certain statistical advantage of utilizing occlusal force measurements, compared to the traditionally adopted optimality criteria approaches that are basically driven by minimizing the energy consumption of muscle systems engaged. Therefore, it is speculated that mastication may not be optimally controlled, in particular for maxillofacially reconstructed patients. For the abnormal muscular system in the patient with orofacial reconstruction, the study shows that in general, the magnitude of muscle forces fluctuates over the 28-month rehabilitation period regardless of the decreasing trend of the maximum muscular capacity. Such finding implies that the reduction of the masticatory muscle activities on the resection side might lead to non-physiological oral biomechanical responses, which can change the muscular activities for stabilizing the reconstructed mandible.

    The aims of this study were to measure the three-dimensional (3-D) loads exerted on the implants supporting an overdenture during function in vivo and to clarify the effect of the number of the implants on the loads. The load-measuring devices were mounted on the implants inserted in the mandible of a 62 aged female subject. Either two or four implants connected with a bar were used for supporting her overdenture. The loads on the implants were expressed three dimensionally as load vectors and were analyzed in the coordinates based on the Frankfort horizontal plane and sagittal plane. The tasks examined were maximum voluntary clenching (MVC) and gum chewing (GC). The total load exerted on the implants with a four-implant supported overdenture (4I-Od) was larger than that with a two-implants supported overdenture (2I-Od) at MVC. The maximum occlusal force on the overdenture showed the same manner (p<0.01). The directions of the loads were posterior and lateral at MVC. The total load exerted on the implants located on the working side, was larger than that on the implants located on the non-working side during GC in the case of 4I-Od. The similar manner was also observed in the 2I-Od. The tensile loads were registered during GC. The functional loads exerted on the individual implants in the case of 4I-Od were smaller than those in the 2I-Od. The loads on the implants during function are thus affected by the number of implants supporting the overdenture.

    顎関節症の主要症候の一つである咀嚼筋群の筋・筋膜痛は,トリガーポイントの存在や,圧痛の局在などの特徴があり,これは筋内部が複雑な多羽状構造を有すること,かつ高度に機能分化していることなどに起因すると考えられている.しかし,咀嚼筋群の筋内部構造,または機能分化については未だ不明な点が多く,局所筋痛発現の原因は解明されていない.これは筋内部構造に対する適切な解析手法が欠如していることに起因する.
    拡散テンソル画像(DTI:Diffusion tensor imaging)は,組織内の水分子の拡散方向を探知することで生体内の線維状走行を捉える手法であり,神経線維のみならず現在では四肢の骨格筋線維の走行解析に応用されている.当分野では,このDTIを咬筋の筋線維形態を解析に応用し,咬筋筋線維描出に適切なDTI撮像条件を同定し,咬筋筋線維走行を非侵襲的に可視化する手法を報告した.さらに異なる下顎位における咬筋内部の筋線維の配向性の変化様相を明らかにし,加えてDTIによる筋線維描出においては被験者毎に適切な解析条件の設定が必要であることを考察した.
    そこで今年度は,これまで咬筋にて検討したDTIによる筋線維描出手法を,より深部に位置する内側翼突筋に応用し,DTIによる筋線維描出を被験者個々で最適にするための解析パラメータの設定について検討を行った.また,その解析条件にて描出した画像をもとに,内側翼突筋の筋線維構造の特徴および異なる下顎位に伴う,その筋線維動態について解析を行った.
    この結果,DTIは深部に位置する内側翼突筋の筋線維を描出する手法として有用であることが示唆された.また,被験者毎に筋内部の水分子の拡散程度が異なるためと推察された.また,個体内でも個体間でも筋線維の配向性のばらつきが認められ,多羽状構造や顎顔面形態の違いがその要因として推察された.

    本研究は,ヒト口腔内における噛みしめや咀嚼などの機能時における歯列上の咬合接触状態,ならびに同部位にて発揮される咬合力を調査可能なセンシングデバイスとして,加わる圧縮力に応じて発電量が変化する特性を有する「静電誘導型発電シート」の活用方法を探索した.これまで研究分担者の東工大の土方らが開発した「静電誘導型口腔内発電シート」を改良し,このシートを歯科一般臨床においてマウスガードや,オクルーザルスプリント等にて用いられている樹脂製シートに埋め込み、上下の歯の接触の有無,その咬合接触部位,ならびに同部における咬合力を発電量変化から解析するシステムの構築を試みた.
    現時点では,発電シートを埋め込んだオクルーザルスプリントへ,既知の荷重を負荷するベンチテストを実施し,荷重時の出力電圧を測定し,その出力値の妥当性を検証した.この結果,発電シートを用いた測定装置からの出力値の高い精度が示され,口腔内荷重測定装置としての妥当性が検証されている.
    また,今回の装置におけるセンサノードにおいては,咬合負荷により発生する電流をデジタル信号に変換して無線通信で外部に送信する方式とし,口腔内における安全性を考慮した無線電力供給方式の導入を計画している.このバッテリーレス動作を実現するため,無線通信部の低消費電力化として低電力インパルス無線送信機の利用可能性について検討し,プリント基板に右図のチップとインピーダンスマッチング用素子,コネクタを実装し測定を行い,3kbpsの場合の消費電力は0.34μWと無線による十分な給電の可能性を示した.

    拡散テンソル画像(DTI:Diffusion tensor imaging)は,組織内の水分子の拡散方向を探知することで生体内の線維状走行を捉える手法であり,神経線維のみならず現在では四肢の骨格筋線維の走行解析に応用されている.本研究は,DTI解析を応用し咬筋内部の筋線維走行の3次元的に捉える手法を確立し,さらに種々の下顎運動時におけるその形態変化の詳細を捉えることを目的とする.本研究は,咀嚼筋でみられる局所的な筋痛の発現機序の解明,さらに患者個々の筋痛発症の原因の診断,治療へと臨床展開するための基盤となる基礎研究である.
    上述の通り,これまでDTIを応用した解析は主に脳神経分野で活発に行われてきたため,これらの解析に用いられるソフトウェアもほとんどが脳神経への応用を前提としたものであった.また,骨格筋への応用例は増加してきているが,骨格筋よりも複雑な形態を有す多羽状筋である咬筋を扱った研究は稀有であり,撮像方法も定まってはいない.そこで今年度は,咬筋に適した撮像条件設定を選定し,データ解析手法の確立を行った.DTIを応用して生体内の線維構造解析を行う手法をファイバートラクトグラフィーと呼ぶが,この結果に大きく影響を及ぼす撮像条件設定として,b値,撮像加算回数,傾斜地場軸数があげられる.これらの設定が咬筋筋線維描出にどのような影響を及ぼすかについて,撮像条件に複数のパターンを設け,5名の健常被験者の撮像を行い解析した.解析に関しては,
    咬筋の複雑な線維構造に対応できるよう新たな形態解析ソフトウェアを構築した.
    この結果,過去の解剖学的知見と照らし合わせてもDTIによって得られる線維の線維長および線維方向はおおむね妥当であることが確認された.一方で,咬筋のDTIファイバートラクトグラフィーにおいて,撮像条件の設定が線維長および線維方向の結果に影響を与えることが明らかとなった.

    Bone tissue exhibits metabolic activity in response to the mechanical stress to the bone, resulting in bone resorption, bone addition, or alteration of trabecular structure. Therefore, inappropriate mechanical stress on the oral tissue may be a factor regarding the resorption of peri-implant bone and/or the mandibular bone beneath the denture base. In order to maintain the healthy mandible for a long time, it is necessary to understand the occlusal force applied to the dentition and the force beneath the denture base during daily function.
    This study attempted to develop some sensor system for measuring the load on the oral tissues during the daily function. For this purpose, the method for fixing the sensors to the oral devices, the method for output analysis, and basic technology for making the sensor systems wearable, which were essential for developing a new device for measuring the daily functional force in human mouth.

    Further development of the digital dentistry requires a breakthrough of the intraoral scanner that enables improvement of measurement accuracy and measurement of the subgingival margin.
    The purpose of this study was to develop SS-OCT (Swept Source Optical Coherence Tomography) intraoral scanner with an intraoral probe using a swept source surface emitting laser that enables high-speed wavelength scanning integrated with a MEMS (Micro Electro Mechanical Systems) actuator as a light source. In this study, we confirmed the clinical usefulness of the newly developed device.

    本研究は, 歯科インプラントにおける臨床的偶発症の予防, さらには歯科インプラントの埋入に係るメカノバイオロジカルな反応の把握した顎骨リモデリングシミュレーションの最適化に寄与する. すなわち, 従来の解剖学的な観点を考慮した手法に加えて本シミュレーションを埋入システムに包含することで, より安全で効率的な歯科インプラント治療を実現することが期待される.上記に示す展望の実現に向けて, これまで申請者らが従事してきた研究並びに臨床経験を基に新たな知見を得ることを目標として本課題に携わった. 特に申請書において, 研究目標を年度ごとに細分化してマイルストーンを設置している. 本年度は, 共同研究者の小川らが確立した実験モデルを踏襲して,Wistar系ラットの脛骨部について純チタンのカスタムメイドインプラントを埋入したモデルを用いて解析を行った. 骨強度評価ソフトウェア(MECHANICALFINDER, 計算力学研究センター, 日本製)を使用して, 撮像したラット脛骨CTのDICOMデータより骨形状を抽出し, 有限要素モデル(以下, FEモデル)を作成した. ラット脛骨の皮質骨および海綿骨の材料特性は, 部位特異的な骨梁構造及び材料非線形性を考慮した設定を行うことで骨内領域を従来手法に比べてより実現象の再現性に富んだモデル構築に成功している. 加えて, 本モデルは経時的な骨代謝の観察と対照することでインプラントの骨梁形態変化の評価を行う基盤となるFEモデルである.

    This study combines clinical investigation with finite element (FE) analysis to explore the effects of buccal bone thickness (BBT) on the morphological changes of buccal bone. One patient who had undergone an implant treatment in the anterior maxilla and experienced the buccal bone resorption on the implant was studied. Morphological changes of the bone were measured through a series of CT scans. A 3D heterogeneous nonlinear FE model was constructed based on the CT images, and the in-vivo BBT changes are correlated to the mechanical stimuli. The anterior incisory bone region of this model was then varied systematically to simulate different BBTs. Significant changes in BBTs were observed clinically. The pattern of bone resorption fell into a strong correlation with the distribution of mechanical stimuli onsite. This study revealed that the initial BBT can significantly affect mechanical responses, which consequentially determines the bone remodeling process.

    Powder jet depositions (PJD) is a precise mechanical coating technique that uses ultrafine particles accelerated to several hundred meters per second by a jet flow of a carrier gas to fabricate functional surfaces with high efficiency. PJD is performed at room temperature and atmospheric pressure, which enables it to be used with HA particles for the creation of HA films. The TiO2-HA particles in the deposited film were densely packed, the maximum and average film thickness were about 56&micro;m and 28&micro;m, respectively. There were no significant differences in the maximum and average thickness before and after thermal cycling. Vickers hardness was 371.39+/-20.68 and bonding strength was 14.78+/-4.44. There were also no significant differences in the hardness and the bonding strength before and after thermal cycling. The thickness, hardness and bonding strength of TiO2-HA films were almost equal to that of HA films created by PJD process.

    Non-metal clasp denture (NMCD) has recently been applied widely as a prosthodontic option. This study aimed to evaluate the retentive forces of NMCDs.
    Four kinds of NMCDs; polyamide-nylon resin non-metal clasp denture (PA-NMCD), polycarbonate resin non-metal clasp denture (PC-NMCD), polycarbonate resin non-metal clasp denture with a metal frame (mPC-NMCD), and acrylic resin denture (PMMA-CD), were fabricated. Load exerted on a direct abutment tooth during pulling off the denture vertically from the model were measured using a piezo-electric transducer. The mesio-lateral component of the load on the abutment tooth by mPC-NMCD and by PMMA-CD was significantly larger than that by PA-NMCD and by PC-NMCD. The vertical component of the load, i.e. the retentive force of the thermoplastic resin clasp, by PMMA-CD was significantly largest. It was suggested that the NMCD could not exert adequate retentive force as same as the conventional metal clasp when 0.25 mm of under-cut was applied.

    The aims of this study are to develop a device for in vivo measurement of three-dimensional (3D) loads in the implants, and to further investigate the implant configuration effects on the load distribution under a fixed partial denture (FPD). A 67-year-old female subject with three implants in mandibular molar area (35, 36, 37). The 3D load-measuring device consisted of piezoelectric transducers and an superstructure. The loads on the implants were recorded during the maximum voluntary clenching (MVC) and chewing a piece of chewing gum (GUM). With the 3I and the Br, the loads exerted on each implant were evenly distributed in both test scenarios. The maximum loads of 335.5 N and 277.6 N were detected on the 36-implant in DCBr and MCBr, respectively, and the tensile load was observed on the 35-implant of DCBr during the GUM. The 36-implant exhibited the largest load in DCBr due to the cantilever effect, with significant margins compared to MCBr and 3I during the GUM (p<0.01).

    Aiming to develop dental implant with self-cleaning function, we prepared experimental Ti-Ag alloys and investigated their properties. The mechanical properties of Ti-Ag alloys were suitable for dental prostheses. Especially, the Ti-25%Ag alloy showed higher strength. A polishability of Ti-Ag alloys was superior to that of pure titanium. Ti-Ag alloys with 20% Ag and 25 % Ag inhibited biofilm formation of Streptococcus mutans and Streptococcus sobrinus. The biocompatibility of Ti-Ag alloys was good.

    This study aimed to reveal effects of the number and position of implants which support a partial overdenture (I-POd) on mechanical loads exerted onto the abutment tooth of the denture and on the residual ridge beneath the denture.Two implants were placed at the second premolar (imp1) and the second molar (imp2) position in edentulous area of model. Load onto the abutment tooth was measured by a piezo-electric 3D-force transducer. Load exerted on residual ridge beneath the denture was measured by a tactile sheet-sensor.Measurements were performed in four test conditions as follows: no implants supporting the denture, imp1 supporting the denture, imp2 supporting the denture and imp1 and imp2 supporting the denture.Loads onto the abutment tooth and residual ridge beneath the denture base were significantly associated with the implant number and position of I-POd. Application of implants beneath the denture base contributes to reduce the loads onto the abutment tooth and residual ridge.

    社会的ニーズである審美性を考慮した部分床義歯どして,ノンクラスプデンチャー(以下,NCD)が臨床で用いられている.しかしNCDの設計は,一般に支台歯に支持・把持を強く求めることが少なく,従来の部分床義歯の設計とは大きく異なっている.そのため口腔内での機能時にNCDが如何なる挙動を示すのか,特に支台歯や顎提粘膜にどのような負荷を与えているかに関しては,不明な点が多い.
    そこで本研究では,NCDにおける支台歯や義歯床下粘膜荷重の特徴を明らかにすることを目的として,支台歯および床下粘膜部へ加わる荷重を静力学的に測定可能な装置を開発し,義歯咬合面上に既知の荷重を付加した際の荷重動態について,NCDと一般的な部分床義歯との比較検討を行うこととした.本年度は,前年度に開発した荷重測定装置を用い,各種材料を用いて製作したNCDを用いて荷重測定を行った.
    実験用義歯は,ポリアミドナイロン系樹脂(バルプラスト^<[○!R]>)のみ(以下,PA-NCD),ポリカーボネート系樹脂のみ(以下,PC-NCD),支持・把持要素に金属フレームを用いたポリカーボネート系樹脂(以下,mPC-NCD),および一般的なアクリリックレジン製部分床義歯(以下,PMMA-CD)で製作した計4種類の部分床義歯とした.試料数は各5個とした.測定は定荷重負荷装置にて実験用義歯の人工歯相当部に静的荷重(10,30,50,100N)を段階的に負荷し,各支台歯に加わる荷重および義歯床下粘膜に加わる荷重を測定した.支台歯の歯軸方向に加わる荷重量は,義歯への負荷が50,100Nの場合,PA-NCDとPC-NCDに比較してPMMA-CDにおいて有意に大きかった.義歯床下粘膜荷重は,負荷が50,100Nの場合,mPC-NCD,PMMA-CDに比較してPA-NCDにおいて有意に大きかった.すなわち,NCDでは機能時に義歯床下粘膜に加わる負荷が従来の部分床義歯よりも過大になる可能性が示された.

    The 3-D load-measuring device, comprised of a piezo-electric force transducer and the experimental superstructure, was developed. The devices were tightened with screws into implant-fixtures in the subjects. The 3-D loads during the maximum voluntary cenching (MVC)and biting a piece of paraffin wax (WAX)were registered. The magnitude and direction of the recorded loads were analyzed in the spatial coordinates based on her Frankfort and sagittal planes. The device enabled us to estimate the 3-D loads on t he implants. Load exerted on the implant supporting fixed superstructure during chewing function was larger than that during clenching. The amount of change in the load direction during chewing was larger than that during chewing. It was suggested that the species of teeth or occulusal contact was affect the load direction

    Three-dimensional (3-D) loads exerted on dental implants are important parameters in the biomechanical control of implant-supported fixed prostheses. It is critical to consider not only the load magnitudes but also the load directions during function to obtain a good prognosis of that treatment. In this study, we developed a system for measuring in vivo the 3-D loads exerted on dental implants and for analyzing the loads on the basis of the stomatognathic system coordinates and clarified the effects of the prosthetic parameters on the loads.

    The 3-dimensional (3-D) functional loads exerted on implants are essential factors in the biomechanical control of implant-supported overdentures. Since there have been few data in vivo, parameters which could affect the loads are still unclear. As for as the results of this study, the loads exerted on the implants during function are affected three dimensionally by the number of implants supporting the overdenture.
