    In the absence of international standards, widely differing attitudes and laws, medical and social cultures strongly influence whether and how patients may access their medical records in various settings of care. Reviewing records, including the notes clinicians write, can help shape how people participate in their own care. Aided at times by new technologies, individual patients and care partners are repurposing existing tools and designing innovative, often 'low-tech' ways to collect, sort and interpret their own health information. To illustrate diverse approaches that individuals may take, six individuals from six nations offer anecdotes demonstrating how they are learning to collect, assess and benefit from their personal health information.

    リスクの内面化はどのように行われるのだろうか? これまでの医療人類学におけるリスク研究は、主に専門家により提示されたリスクとその対処法に抗する人々の生き方を描いてきた。一方、本研究は、専門家から提示されるそれらに従順な人々を研究の対象に置く。具体的には、2011年から2016年の5年にわたる循環器専門医療機関でのフィールドワークをもとに、心房細動と呼ばれる不整脈とその治療に伴うリスクが、(1)どのように医師から患者に伝えられ、(2)患者はどのようにそのリスクを理解し、日常を過ごしているのかを明らかにすることを目指す。心房細動に罹患すると、心原性脳梗塞という脳梗塞の中でも広汎で重篤なタイプの脳梗塞に罹患するリスクが上昇する一方、その予防のための治療が、大出血のリスクを上昇させることが知られている。つまり心房細動の治療においてはふたつの拮抗するリスクが存在し、この点で心房細動という疾患は本研究の目的に適している。分析の視座として本研究はナラティヴ・アプローチを用い、とりわけ次の3点に注意を払った。まず一つには、ナラティヴは出来事と出来事をつなぎ合わせるという点、二つには、出来事を作り出すという点、そして三つには、目的を内在し、語り手あるいは聞き手の意図の変容を伴う、という点である。データを分析したところ、医師は直喩と隠喩を駆使したナラティヴを自在に用いながら、エビデンスをナラティヴの参照点の三つとした、価値の体系を患者に伝えていることが判明した。患者の語りにも医師のナラティヴは反映されていたが、その一方で患者のナラティヴには、決して消えることのない心原性脳梗塞のリスクと向き合う困難が刻まれていた。本研究により、エビデンスとナラティヴを二項対立的に捉え、前者を科学的真実、後者を尊重すべきではあるが不十分な知識に形作られた患者の信念と捉えることの弊害が一層明らかになるとともに、患者の未来に危機を作り出しはするが、その危機との対峙の仕方については患者および臨床医に最終的にゆだねてしまう、予防医学の道徳的無関心が明らかになった。Around the 1970s, surveillance medicine̶medicine based on probability statistics and promoting risk management to the public̶became prevalent. Medical anthropologists have attempted to illustrate the complexity of human lives under the influences of surveillance medicine, and their studies mostly focus on those who resist its power. This research, however, focuses on those who adhere to the power of surveillance medicine in order to understand the communication of risks to individuals and how individuals internalize these risks. This study focused on patients with atrial fibrillation and doctors who treated them. This population was chosen because patients with atrial fibrillation usually need to deal with two concomitant risks: brain infarction caused by blood clots, and major bleeding. Atrial fibrillation increases the risk of brain infarction so anticoagulation treatment is often chosen to decrease its risk. Anticoagulation treatment, however, increases the risk of major bleeding including cerebral hemorrhage. This study is based on periodic fieldwork conducted between 2011-2016, including participant observation at a cardiovascular clinic in Tokyo and interviews with patients with atrial fibrillation and their doctors. This research found that doctors employed similes and metaphors to communicate these risks to patients. The use of metaphor in their narratives was especially significant. Japanese doctors often used a Japanese onomatopoetic term, sarasara, which might refer to the murmur of a beautiful stream, to express the condition of blood under the influence of anticoagulation drugs. Although the drugs predispose the patients to bleeding, patients understood their blood condition becoming sarasara as if the medications transformed their blood into a desirable state. We argue that abstract, scientifically calculated risks take on meaning by being embedded in a narrative laden with cultural values̶a narrative that complicates the very notion that "risk" itself is calculable.

    There is a growing interest in qualitative research among Japanese clinicians. Many of those who are interested in qualitative research attempt to identify the best method to guide them throughout their research. However, good qualitative research never arises from a method but a researcher's way of looking at reality. This paper, therefore, is based on cultural anthropology that has a long history of conducting qualitative research and illustrates three philosophical attitudes that leverage these clinicians for generating significant qualitative research results. These are: (i) looking at the reality from the perspective of informants, (ii) suspending researchers' ethics and their attempts to control people, such as patients and their families, and (iii) aiming at discovering not the average of the collected qualitative data but a new perspective that can only be found in the research field.

    Background: The etiological model of eating disorders which asserts that inappropriate parenting results in the development of an eating disorder, which this study calls "the familial model," has already received much criticism. Although many Japanese medical professionals usually attempt to explain that the familial model is wrong because of its low scientific probability, the model still has a strong impact on patients with eating disorders and their families in Japan. To explain why many patients employ the familial model to understand their eating disorders in spite of the criticism, this study uses for its theoretical framework not a natural science method but an approach based on the narratives of the illness. Subjects and Method: This study conducted a qualitative interview with an anorexic patient. The interviews were conducted 14 times over a period of 27 months. Since this subject had internalized the familial model, this study analyzed how this internalization had occurred and how the patient interpreted the etiology of her anorexia nervosa. Result: The subject internalized the familial model through medical consultations, professional books about eating disorders, and her parents' attitudes, which drastically changed after her diagnosis. The subject continued to suffer severe anorexia as she faced difficult problems in her life. Indeed, the anorexic condition helped her to escape from these problems. The subject explained that she became anorexic because of inappropriate parenting from her parents who, for instance, expected too much from and exercised excessive control over their children ; therefore, she was only able to explain her psychological pain by starving herself. However, she started to recover from her anorexia after she succeeded in making social relationships outside her family. Conclusion: This study finds that the familial model is able to restore psychological coherence to the past and present of a patient disturbed by the development of an eating disorder. The study also shows how patients' guilt feelings about developing, having, and failing to recover from an eating disorder are alleviated by blaming their parents. Therefore, this study finds that through the internalization of the familial model the patient sees some gain from her illness. However, this gain at the same time results in inhibiting the recovery of their social skills, since through blaming their parents as a cause, they are allowed to escape into an eating disorder rather than confronting the real problems that they failed to handle in the process of developing their disorder.

    BACKGROUND: Incest is a globally prevalent phenomenon and is defined as sexual relations between closely related family members. Despite being a criminal offense irrespective to the gender and age incest is a prevalent issue in Nepal. Incest survivors are sheltered in safe homes for specific periods and reintegrated into their families following their healing. This research aims to explore the challenges and issues involved in the reintegration of incest survivors and investigate how the interconnected perceptions of families and individuals relate to the incest survivors' successful reintegration. METHODS: This qualitative study was conducted in three districts of Nepal. Fourteen incest survivors, five service providers from safe homes, and 22 community members were purposefully and theoretically selected. Semi-structured interviews were carried out to collect the study data, which were subsequently coded and analyzed using a grounded theory approach. RESULTS: The results indicate the importance of the reintegration of incest survivors, as staying in a safe home may not offer a long-term solution for these individuals. However, reintegration is challenging without acceptance and support from the survivors' families and communities, the survivors' readiness to reintegrate, and agreement from the survivors' safe homes. Factors found to influence the unsuccessful reintegration of survivors included family fragmentation, socio-economic status, family resistance, the survivor-perpetrator relationship, survivors' interests, fears, and the possibility of repeated incidents, harmful social norms and cultural practices, community intervention, and negative perceptions. CONCLUSION: This study found that factors associated with impeding the reintegration of incest survivors are found in survivors, their families, and their communities. Creating harmonious family systems, providing education on gender equality, empowering women to fight against all sorts of abuse, and governmental assurances on the implementation of legal aid, human rights, ratified conventions on child rights and women's rights may help prevention of incest, and address the reintegration challenges of incest victims.
