    AIM: The aim of the study was to identify the clinical significance of anxiety in those with depression, the Diagnostic and Statistical Manual of Mental Disorders 5th edition (DSM-5) defined criteria for an anxious distress specifier for major depressive disorder (MDD). The Clinically Useful Depression Outcome Scale (CUDOS) supplemented with questions for the DSM-5 anxious distress specifier (CUDOS-A) is a self-report instrument to assess the clinical significance of anxiety in addition to assess symptoms and the severity of depression. This study aimed to evaluate the psychometric properties of the Japanese version of the CUDOS-A. METHODS: An observational, prospective study was conducted with 131 MDD outpatients and 200 healthy controls. The Japanese version of the CUDOS-A, along with other measures, was administered to assess depressive symptoms, anxiety, social function, and biological rhythm. Reliability and validity analyses were performed, including internal consistency, test-retest reliability, convergent validity, and contrasted-groups validity. RESULTS: The Japanese version of the CUDOS-A demonstrated excellent internal consistency (Cronbach's alpha = 0.96) and test-retest reliability (ICC = 0.78). Significant positive correlations were found between the CUDOS-A and measures of depression, anxiety, social function, and biological rhythm (all, p < 0.001), supporting its convergent validity. The CUDOS-A effectively differentiated between patients with MDD and healthy controls (p < 0.001), indicating good contrasted-groups validity. CONCLUSIONS: The Japanese version of the CUDOS-A is a useful measure for research and for clinical practice, enabling the efficient assessment of anxious distress in individuals with depression.

    Sleep timing is an important output of the circadian system. The COVID-19-mandated social restrictions significantly altered commuting time and sleep duration regionally in Japan. This study aimed to elucidate sleep patterns, especially chronotype and social jetlag (SJL), due to changes in social time pressure through the social restrictions between the Metropolitan and Regional areas in Japan. As part of the Global Chrono Corona Survey 2020 (GCCS), the data were collected during social restrictions (SR), but pre-COVID-19 behaviours were also queried retrospectively. We analyzed a cohort of 729 respondents representing both the Metropolitan and the Regional areas separately for workdays and work-free days. While the areas showed no difference in SJL before SR, the differential decrease was larger in the Metropolitan area during SR, resulting in a significant difference in SJL between the areas. The outdoor light exposure before SR was 30 min longer in the Metropolitan areas than in the Regional; during SR both areas showed similarly low (below 1 h) outdoor light exposures. The variables associated with decreased SJL were the Metropolitan areas, work-from-home, a no-usage alarm clock on workdays, and chronotypes (mid-sleep time on free days corrected for sleep deficit accumulated over the workweek, MSFsc) during SR. The results suggest that relaxed social schedules, as reflected in the increased frequency of work-from-home and reduced alarm clock use, and moving towards earlier MSFsc during SR were linked to decreased SJL and were more prominent in the Metropolitan areas. This study provides insights into sleep patterns and the social time pressure markers, by comparison between residential groups in Japan.

    BACKGROUND: Pregnancy-related anatomic, physiologic, and hormonal factors can occur at different stages of pregnancy and affect sleep disturbances. Sleep problems and habits among Japanese women across gestational stages have not been described. This study aimed to elucidate sleep problems, including a high risk of sleep-disordered breathing and sleep habits, among Japanese women across gestational progress, including postpartum. METHODS: A questionnaire survey using the Pittsburgh Sleep Quality Index (PSQI) and Berlin Questionnaire was conducted for 683 pregnant women (223 in the second trimester, 453 in the third trimester) and 386 postpartum (within 1 week after delivery). RESULTS: The total PSQI score was 5.3 [2.6] in the second trimester, 6.1 [2.8] in the third trimester, and 6.8 [3.1] in the first week postpartum. The percentage of those at high risk for sleep-disordered breathing, as determined by the Berlin Questionnaire, was 11.8% in the second trimester, 21.3% in the third trimester, and 19.2% in the first week postpartum, with the highest percentage of those at high risk in the third trimester of pregnancy. CONCLUSION: The PSQI total score exceeded the cut-off value in the third trimester and the first week postpartum, suggesting sleep deterioration. Therefore, it is necessary to understand and support sleep in pregnant women to ensure safe delivery, postpartum recovery, and health.

    Delayed sleep-wake phase disorder (DSWPD) is a sleep disorder in which the habitual sleep-wake timing is delayed, resulting in difficulty in falling asleep and waking up at the desired time. Patients with DSWPD frequently experience fatigue, impaired concentration, sleep deprivation during weekdays, and problems of absenteeism, which may be further complicated by depressive symptoms. DSWPD is typically prevalent during adolescence and young adulthood. Although there are no studies comparing internationally, the prevalence of DSWPD is estimated to be approximately 3% with little racial differences between Caucasians and Asians. The presence of this disorder is associated with various physiological, genetic and psychological as well as behavioral factors. Furthermore, social factors are also involved in the mechanism of DSWPD. Recently, delayed sleep phase and prolonged sleep duration in the young generation have been reported during the period of COVID-19 pandemic-related behavioral restrictions. This phenomenon raises a concern about the risk of a mismatch between their sleep-wake phase and social life that may lead to the development of DSWPD after the removal of these restrictions. Although the typical feature of DSWPD is a delay in circadian rhythms, individuals with DSWPD without having misalignment of objectively measured circadian rhythm markers account for approximately 40% of the cases, wherein the psychological and behavioral characteristics of young people, such as truancy and academic or social troubles, are largely involved in the mechanism of this disorder. Recent studies have shown that DSWPD is frequently comorbid with psychiatric disorders, particularly mood and neurodevelopmental disorders, both of which have a bidirectional association with the pathophysiology of DSWPD. Additionally, patients with DSWPD have a strong tendency toward neuroticism and anxiety, which may result in the aggravation of insomnia symptoms. Therefore, future studies should address the effectiveness of cognitive-behavioral approaches in addition to chronobiological approaches in the treatment of DSWPD.

    BACKGROUND: Delayed sleep-wake phase disorder (DSWPD) is more prevalent among the younger generation. However, the prevalence of this disorder in Asia, particularly Japan, has not yet been elucidated. Furthermore, the impact of DSWPD morbidity on daytime functioning and factors associated with the presence of the disorder remain unclear. METHODS: A web-based survey was conducted among youth aged 15-30 years. In total, 7,810 individuals completed the questionnaire. The questionnaire included items on sociodemographic variables as well as the Japanese version of the Biological Rhythms Interview of Assessment in Neuropsychiatry self-report (J-BRIAN-SR), which assesses the risk of DSWPD, sleep behaviors and possibly related lifestyle variables, productivity loss [WHO Health and Work Performance Questionnaire (HPQ)], and health-related quality of life (HRQOL). The risk of DSWPD was indicated by a J-BRIAN-SR score greater than or equal to 40 points and days of absence ≥4 days per month. After comparing these variables for participants at risk of DSWPD and those who were not, the factors associated with the risk of DSWPD were examined using logistic regression analysis, with sociodemographic and lifestyle variables as independent variables. RESULTS: The overall prevalence of participants at risk DSWPDs was 4.3%. Compared with participants without DSWPD, those at risk of DSWPD presented significantly worse HPQ and HRQOL scores. The risk of DSWPD was positively associated with the presence of currently treated diseases, length of nighttime liquid crystal display (LCD) viewing, and being a high school/university students. It was negatively associated with habitual exercise. CONCLUSION: The risk of DSWPD seemed to be consistent with reports from Western countries, and individuals possibly affected by the disorder were thought to have deteriorated daytime functioning. In addition, lifestyle specific to youth, such as long-term LCD viewing at night and relatively loose social constraints, could be associated with the presence of DSWPD in this generation.

    This web-based cross-sectional survey aimed to elucidate the differences between the two core symptoms of night eating syndrome (NES): evening hyperphagia and nocturnal ingestion in the general Japanese population aged 16-79 years. Participants who consumed at least 25% of daily calories after dinner were defined as having evening hyperphagia. Those who consumed food after sleep initiation at least twice a week were determined to have nocturnal ingestion. Of the 8348 participants, 119 (1.5%) were categorized in the evening hyperphagia group, 208 (2.6%) in the nocturnal ingestion group, and 8024 in the non-NES group. Participants with evening hyperphagia and nocturnal ingestion had significantly higher anxiety scores (p < 0.05 and p < 0.001, respectively) and depression (p < 0.001 for both) than those without NES. Multiple logistic regression analysis revealed that evening hyperphagia was significantly and independently associated with higher body mass index, shorter sleep duration, later sleep-wake schedule, and higher insomnia score, while nocturnal ingestion was significantly and independently associated with younger age, smoking habit, living alone, earlier sleep-wake schedule, and higher insomnia score. Sleep duration and sleep-wake schedule characteristics in the two groups were opposite, suggesting differences in the sleep pathophysiology mechanisms.

    Light is an important regulator of daily human physiology in providing time-of-day information for the circadian clock to stay synchronised with the 24-hr day. The coronavirus disease 2019 (COVID-19) pandemic led to social restrictions in many countries to prevent virus spreading, restrictions that dramatically altered daily routines and limited outdoor daylight exposure. We previously reported that sleep duration increased, social jetlag decreased, and mid-sleep times delayed during social restrictions (Global Chrono Corona Survey, N = 7,517). In the present study, we investigated in the same dataset changes in wellbeing and their link to outdoor daylight exposure, and sleep-wake behaviour. In social restrictions, median values of sleep quality, quality of life, physical activity and productivity deteriorated, while screen time increased, and outdoor daylight exposure was reduced by ~58%. Yet, many survey participants also reported no changes or even improvements. Larger reductions in outdoor daylight exposure were linked to deteriorations in wellbeing and delayed mid-sleep times. Notably, sleep duration was not associated with outdoor daylight exposure loss. Longer sleep and decreased alarm-clock use dose-dependently correlated with changes in sleep quality and quality of life. Regression analysis for each wellbeing aspect showed that a model with six predictors including both levels and their deltas of outdoor daylight exposure, sleep duration and mid-sleep timing explained 5%-10% of the variance in changes of wellbeing scores (except for productivity). As exposure to daylight may extenuate the negative effects of social restriction and prevent sleep disruption, public strategies during pandemics should actively foster spending more daytime outdoors.

    AIM: The aim of this study was to investigate the impact of sleep problems on job stress in office workers. METHODS: This study included 4645 office workers from 29 companies who completed the study questionnaires between April 2017 and April 2019 in Japan. Sleep duration was assessed based on the participants' subjective sleep schedule on workdays and free days. The midpoint of sleep on free days (sleep-corrected) and social jetlag were calculated in accordance with the Munich Chronotype Questionnaire. To assess job stress, we used the 57-item Brief Job Stress Questionnaire. RESULTS: Multivariate logistic regression analysis revealed that the following factors were significantly associated with high job stress in office workers: a sleep duration <6 h on workdays (OR = 1.77, 95% CI = 1.46-2.15, P < 0.001), a sleep duration <6 h on free days (OR = 1.40, 95% CI = 1.05-1.87, P = 0.022), a sleep duration of at least 8 h on free days (OR = 1.31, 95% CI = 1.06-1.60, P = 0.011), and more than 2 h of social jetlag (OR = 1.33, 95% CI = 1.04-1.70, P = 0.022). CONCLUSION: This study suggests that insufficient sleep, long sleep durations on free days, and social jetlag may be associated with high job stress in office workers.

    OBJECTIVE: The Biological Rhythms Interview of Assessment in Neuropsychiatry (BRIAN) might be applicable for assessing delayed sleep-wake phase disorder (DSWPD). We aimed to investigate the reliability and validity of the Japanese version of the BRIAN self-report (J-BRIAN-SR) in DSWPD patients and determine a cutoff score to identify the presence of the disorder. METHODS: We enrolled 60 newly diagnosed DSWPD outpatients and 64 age-matched healthy controls. We used Cronbach's alpha for internal reliability to evaluate J-BRIAN-SR. We confirmed the reliability of the A test and re-test using Pearson's correlation coefficient in the controls. We used confirmatory factor analysis to evaluate the factor structure of J-BRIAN-SR and referenced the Morningness-Eveningness Questionnaire (MEQ) to check concurrent validity. We analyzed the receiver operating characteristic curve (ROC) to determine the J-BRIAN-SR cutoff point for the presence of DSWPD. RESULTS: The 18-component scores of the J-BRIAN-SR had an overall reliability coefficient (Cronbach's alpha) of 0.82. We confirmed a high test-retest reliability using an intraclass correlation coefficient (r = 0.84). The correlation between J-BRIAN-SR and MEQ was 0.38 (p = 0.003). The J-BRIAN-SR that we extracted by exploratory factor analysis consisted of three factors. A score of 40 points provided a sensitivity of 80.0% and a specificity of 75.6% for the positivity of DSWPD. CONCLUSIONS: The results of the present study revealed that J-BRIAN-SR is a valid and reliable instrument for screening and evaluating the severity of DSWPD. Our findings will be useful to physicians and patients in Japan and those in clinical settings.

    The aim of the present study was to investigate the association among lunar cycle, menstrual cycle onset, and subjective sleep quality. Menstrual cycle onset data from the six most recent menstrual cycles were obtained for 529 women (aged 25-39 years) using the smartphone app Luna Luna. We also collected questionnaire survey data on sleep quality from each participant. Overall, there was no association between the onset of menstrual cycle and lunar phase. Interestingly, the proportion of good sleepers with menstrual cycle beginning during the light period was significantly higher than that during the dark period, while the proportion of poor sleepers with menstrual cycle beginning during the dark period was significantly higher than that during the light period. When participants were categorized by the combination of lunar phases (light, dark, neutral periods) in the two most recent menstrual cycle onsets, the "both dark period" group and the "other (light and dark) period" group showed the lowest proportion of good sleepers. Menstrual cycle onset in the dark period was associated with a deterioration in subsequent subjective sleep quality, which was more apparent with consecutive onsets in the dark period or at a rapidly changing lunar phase.

    Although sleep debt and social jetlag (SJL) influence daytime dysfunctions, the effects of both sleep debt and SJL on them have not been analyzed. The aim of this study was to examine the mutual relationship between sleep debt and SJL on daytime sleepiness, mood, and work performance. This study was a cross-sectional study on sleep health conducted on the Japanese general population. A total of 4505 general workers (30% female, aged 43.57 ± 11.63 years) were selected and analyzed. Sleep debt was defined by sleep debt index (SDI), which is the discrepancy between desired and real sleep duration. SJL and SDI scores exhibited a positive but weak coefficient (r = 0.19). In a 4 (SJL) × 3 (SDI) two-way ANOVA, the interaction effects were notable for sleepiness and depression scores, while the group effects were notable for the work performance score. For sleepiness and depression scores, SDI >2 h was not significantly different from SJL. In addition, the impact of SDI was higher than that of SJL on sleepiness (β = 0.17), depression (β = 0.16), and work performance (β = -0.10). The impact of sleep debt was more pronounced than SJL on daytime dysfunctions, although both sleep debt and SJL have negative impacts on them.

    Excessive daytime sleepiness is increasingly being recognized as a major global health concern. However, there have been few studies related to sleepiness and its associated factors in elementary school children. In Japan, all schools were closed from February to May 2020 to prevent coronavirus disease 2019 (COVID-19) outbreaks. The aim of this study was to identify changes in the subjective sleepiness of pupils during the 1.5-year period and to elucidate factors associated with changes in sleepiness. Questionnaire surveys about pupils' sleep habits and the Japanese version of the Pediatric Daytime Sleepiness Scale (PDSS-J) were conducted longitudinally at one elementary school in June 2019, January 2020, and June 2020. The average ∆PDSS score was 0.94 ± 5.51 (mean ± standard deviation) from June 2019 to January 2020 and -1.65 ± 5.71 (t[498] = 6.13, p < 0.01) from January 2020 to June 2020. Univariate and multivariate logistic regression analyses revealed that decreasing social jetlag was associated with decreasing PDSS scores (OR = 0.77, 95% CI: 0.62-0.96, p = 0.02) during the school closure. A less restrictive school schedule secondary to a COVID-19-related school closure decreased sleepiness in children and was associated with decreasing social jetlag.

    In humans, sleep regulation is tightly linked to social times that assign local time to events, such as school, work, or meals. The impact of these social times, collectively-social time pressure, on sleep has been studied epidemiologically via quantification of the discrepancy between sleep times on workdays and those on work-free days. This discrepancy is known as the social jetlag (SJL). COVID-19-mandated social restrictions (SR) constituted a global intervention by affecting social times worldwide. We launched a Global Chrono Corona Survey (GCCS) that queried sleep-wake times before and during SR (preSR and inSR). 11,431 adults from 40 countries responded between April 4 and May 6, 2020. The final sample consisted of 7517 respondents (68.2% females), who had been 32.7 ± 9.1 (mean ± sd) days under SR. SR led to robust changes: mid-sleep time on workdays and free days was delayed by 50 and 22 min, respectively; sleep duration increased on workdays by 26 min but shortened by 9 min on free days; SJL decreased by ~ 30 min. On workdays inSR, sleep-wake times in most people approached those of their preSR free days. Changes in sleep duration and SJL correlated with inSR-use of alarm clocks and were larger in young adults. The data indicate a massive sleep deficit under pre-pandemic social time pressure, provide insights to the actual sleep need of different age-groups and suggest that tolerable SJL is about 20 min. Relaxed social time pressure promotes more sleep, smaller SJL and reduced use of alarm clocks.

    Several studies have assessed the effects of milk and dairy product intake on sleep quality and duration. Such investigations have varied in terms of their geographic locations, amounts of milk and dairy products, study participants (age, sex, race), and study designs. The present study aimed to summarize this literature and provide a unified view on whether the intake of milk and dairy products affects sleep quality. This systematic review was conducted according to the preferred reporting items for systematic reviews and meta-analyses (PRISMA) guidelines. The following keywords were chosen as electronic database search items from MeSH (medical subject headings) terms and descriptors in health sciences (DeHS) lists: milk, yogurt, dairy product, cheese, sleep, human, observational study, and interventional study. As a result, a total of 14 studies published between 1972 and 2019 were included in this review, including eight randomized controlled trials, two experimental studies with cross-over designs, one longitudinal study, and three cross-sectional studies. Four studies targeted older adults, three included toddlers, two targeted children, and six enrolled adults inclusive of university students. Overall, these studies indicated that a well-balanced diet that includes milk and dairy products is effective in improving sleep quality, despite mixed results across studies attributable to differences in study populations and methods.

    BACKGROUND: To date, no previous studies have evaluated the relationship between sleep duration and quality of life (QOL) or depression in the general population after controlling for daytime sleepiness and sleep disturbances. METHODS: A web-based cross-sectional survey was conducted with 8698 subjects aged 20-69 years. We examined the relationships between weekday sleep duration and daytime sleepiness, sleep disturbance, QOL and depression, using the Epworth Sleepiness Scale, Pittsburgh Sleep Quality Index (without the item for sleep duration), 8-item Short Form and Center for Epidemiological Studies Depression Scale (CES-D). RESULTS: Daytime sleepiness tended to increase in proportion to shorter weekday sleep durations. Sleep disturbances, physical and mental QOL, and CES-D scores were worse in both the shorter and longer sleep groups compared with the group with 7-8 h of sleep. Hierarchical logistic regression analyses revealed that short sleep duration but not long sleep duration was significantly associated with reduction of both physical and mental QOL, even after controlling for the presence of daytime sleepiness and sleep disturbance. Both short and long sleep duration were independently and significantly correlated with depression after controlling for daytime sleepiness; however, there was no statistically significant association after adjusting for the effects of sleep disturbance. CONCLUSIONS: The results suggested adverse effects of short sleep but not long sleep on both physical and mental QOL. In addition, the negative impact of specific types of sleep disturbance on depression may be greater than the impact of shortening of sleep duration.

    Clock genes that comprise the circadian clock system control various physiological functions. Delayed sleep-wake phase disorder (DSWPD) and night eating syndrome (NES) are characterized by delayed sleep and meal timing, respectively. We estimated that clock gene expression rhythms in DSWPD patients may be delayed in comparison with the healthy subjects due to delayed melatonin secretion rhythms, producing eveningness chronotype in these individuals. However, it was difficult to estimate which clock gene expression rhythms were delayed or not in NES patients, because previous studies revealed that melatonin secretion rhythm was a little delayed compared with healthy individuals and that chronotype of NES patients depended on the individuals. Therefore, we examined expression rhythms of clock genes such as Period3 (Per3), nuclear receptor subfamily 1, group D, member 1 (Nr1d1) and Nr1d2 in these patients. Further, we expected sleep and meal patterns in DSWPD and NES patients may be more diverse than patterns observed in healthy subjects, and thus analyzed relationships among clock gene expression rhythms, sleep quality, sleep midpoint time, and meal times. We enrolled healthy male participants along with DSWPD and NES male patients, and asked all participants to answer questionnaires and to keep diaries to record information on their sleep and meals. Further, we asked them to collect 5-10 beard follicle samples, 6 times every 4 h. We measured clock gene expression rhythms using total RNA extracted from beard follicle cells. Peak time of clock gene expression in the NES group showed more diversity than the other groups, and that in the DSWPD group was delayed compared with the control group. In addition, the peak time of clock gene expression was negatively correlated with sleep quality and positively correlated with meal time after a long fast. Amplitudes of clock gene expression, especially Per3, positively responded to better mental and physical conditions as well as with better sleep quality. Results of this study suggest that peak times of clock gene expression in NES patients depended on the individuals; some patients with NES showed similar clock gene expression rhythm to healthy subjects, and other patients with NES showed similar to DSWPD patients. Moreover, this study suggests that meal time after a long fast may influence more determination in clock gene expression rhythms than the time of breakfast. Therefore, this study also indicates that Per3 clock gene may be one of the parameters that will help us understand sleep and meal rhythm disturbances.

    Menstrual symptoms may have a significant impact on women's lives. Many women experience menses-related health problems, such as menstrual pain, heavy menstrual bleeding, and premenstrual syndrome, during their reproductively fertile years. Circadian misalignment in shift workers has been reported to contribute to menstrual cycle irregularity and/or painful menstruation. However, the relationship between social jetlag (SJL) and menstrual symptoms/menstrual cycle has not been elucidated. In this study, we aimed to elucidate this relationship among female university students. One-hundred and fifty female university students (mean [SD]: 18.8 [0.71]-years old) completed self-reported questionnaires consisting of menstrual symptoms and menstrual cycle, sleep quality and sleep habits, quality of life, and demographic variables. The average menstrual cycle was 32.0 [5.4] days. The percentage of students who showed menstrual cycle irregularity, having less than 25 days or more than 39 days of menstrual cycle during the previous four menstrual cycles, was 60.6%. SJL, the difference between mid-sleep time on free days and mid-sleep time on school days, was categorized into small (absolute SJL < 1 h) or large (≥1 h). Overall, 78.0% of participants had SJL ≥ 1 h. Among the menstrual symptoms, pain, behavioral change, and water retention subscale scores were significantly higher in the SJL ≥ 1 h group than in the SJL < 1 h group. However, no significant differences were found in concentration, autonomic reaction, or negative affect subscale scores between the two groups. The menstrual cycle was 31.2 [5.5] days in the SJL < 1 h group and 32.2 [5.4] days in the SJL ≥ 1 h group, without significant difference. Logistic regression analysis showed that more than 1 h of SJL was a significant associated factor with severe menstrual symptom, independently of sleep duration and late chronotype. This study indicated that SJL was a significant factor associated with severe menstrual symptoms, suggesting the possibilities of association between circadian system and reproductive function among humans.

    PURPOSE: The aim of this study was to ascertain the clinical characteristics of drug-induced sleep-related eating disorder (SRED). PATIENTS AND METHODS: We retrospectively reviewed the medical records of 30 patients with primary SRED (without any comorbid sleep disorders and who were not taking any possible causative medications), and ten patients with drug-induced SRED (occurrence of SRED episodes after starting nightly medication of sedative drugs, which completely resolved after dose reduction or discontinuation of the sedatives). RESULTS: All patients with drug-induced SRED took multiple types of sedatives, such as benzodiazepines or benzodiazepine receptor agonists. Clinical features of drug-induced SRED compared with primary SRED were as follows: higher mean age of onset (40 years old in drug-induced SRED vs 26 years old in primary SRED), significantly higher rate of patients who had total amnesia during most of their SRED episodes (75.0% vs 31.8%), significantly lower rate of comorbidity of night eating syndrome (0% vs 63.3%), and significantly lower rate of history of sleepwalking (10.0% vs 46.7%). Increased doses of benzodiazepine receptor agonists may be responsible for drug-induced SRED. CONCLUSION: The clinical features of drug-induced SRED were different from those of primary SRED, possibly reflecting differences in the underlying mechanisms between these two categories of SREDs.

    Objective The aim of this study is to reveal factors associated with both dozing off at the wheel and motor vehicle accidents (MVAs) with sleep apnea syndrome (SAS) as well as other descriptive variables for driving set as independent factors.<br/>Methods We conducted an anonymous questionnaire survey on drivers who visited the Tokyo Metropolitan Driver and Vehicle Licensing Center in order to renew their driver's license from September 25 to 29, 2006. The survey requested information regarding self reported experience of dozing off at the wheel, MVAs, usual mileage, length of license holding period, and the participants' awareness of SAS. We obtained data from 3,236 drivers (response rate: 61.8%).<br/>Results The rates for previous experience of feeling sleepiness at the wheel, that of dozing off at the wheel, and that of near-miss or actual MVA due to dozing off at the wheel among the participants were 40.4%, 20.3%, and 10.4% respectively. Multivariate logistic regression analysis revealed that dozing off at the wheel was significantly associated with male gender, long-term holding of a driver's license (≥3,<10 years: OR=1.58, 95%CI: 1.04–2.42, ≥10,<20 years: OR=2.23, 95%CI: 1.47–3.38, ≥20,<30 years: OR=1.89, 95%CI: 1.23–2.90, ≥30 years: OR=1.85, 95%CI: 1.18–2.89), higher usual mileage (≥30 km per a week: OR=1.35, 95%CI: 1.06–1.73), appearance of sleepiness shortly after starting driving (<2hrs: OR=1.99, 95%CI: 1.60–2.47), either having awareness or a diagnosis of SAS. As to experience of near-miss or actual MVA due to dozing off at the wheel, factors including male gender, long-term holding period of driver's license (≥3,<10 years: OR=2.02, 95%CI: 1.15–3.55, ≥10,<20 years: OR=2.10, 95%CI: 1.19–3.70), appearance of sleepiness shortly after starting driving (<2hrs: OR=1.70, 95%CI: 1.30–2.24), either having awareness or diagnosis of SAS were significantly associated.<br/>Conclusion Not only habituation effects and fatigue due to long driving but also SAS have a significant impact on the occurrence of sleepiness related MVAs. Moreover, early diagnosis and treatment of SAS is necessary so that we can avoid dozing off at the wheel, resulting in near-miss or actual MVA.

    Several epidemiological studies have shown that sleep problems are common in Japanese general population. When treating sleep disorders, it is important to aim both at improving the disorder as well as often related dysfunctional daily life patterns. Recent studies have been designed specifically to evaluate the impact of sleep disorders on QOL (quality of life). The objective of this paper is to review the influence on social life of insomnia and insufficient sleep, which are the most common sleep disorders. Insomnia is statistically linked to a worsening of health condition. Persistent insomnia increases the risk of several somatic diseases such as hypertension and diabetes mellitus. In addition, the disorder has also a negative impact on waking functions related to quality of life, e.g. impaired concentration, impaired memory, decreased ability to accomplish daily tasks, and decreased enjoyment of interpersonal relationships. The risk of both work-related accidents and motor-vehicle accidents are usually found to be higher in insomniacs than in controls. Moreover, several studies have provided data suggesting that insomnia is one of the risk factors for the development of depression. Although chronic insufficient sleep and sleep loss are highly frequent in Japanese population, the number of insufficient sleepers who consult medical facilities is thought to be relatively low. From our study, 22% of the patients with insufficient sleep syndrome reported having had accidents or near misses. Moreover, the group of patients reporting these mishaps showed higher sleepiness scores than the other patients. This finding leads us to emphasize that insufficient sleep should not be overlooked and should be treated by prolonging nocturnal sleep and/or taking an appropriate daytime nap. Substantial treatment for sleep disorders usually improves not only sleep itself but also QOL. The importance of early recognition and treatment of sleep disorders as well as campaigns for sleep health awareness must be emphasized.

    It is well known that intestinal motility has circadian rhythms as well as sleep-wake cycles and muramyl peptides affect sleep as the sleep-inducing substance. It is possible that bowel habits influence sleep health. The aim of this study is to epidemiologically investigate the relationship between bowel habits including functional constipation (FC) and irritable bowel syndrome (IBS), and sleep health. Young adult women living in the Tokyo metropolitan area were examined using the selfreported questionnaire. Subjects with FC or IBS were selected by Rome II. The score estimating the degree of parasomnia-like problems in FC and IBS was higher than in control. Weekday sleepiness in FC and IBS was also higher than in control. Weekday sleeping time in IBS was significantly shortened. The number of having breakfast, time cue of biological rhythms, was extremely decreased in FC. Irregularity of sleep onset time, sleeping time and sleep habits, which are related to biological rhythms, in FC and IBS were higher than in control. This study suggests that sleep health in FC and IBS is impaired compared with control and bowel habits may be related to sleep health. Bowel habits seem to be worthy of notice as a risk factor for sleep disorders.

    Since intestinal motility has circadian rhythms as well as sleep-wake cycles, it is possible that there are some relationships between functions of the gastrointestinal tract, especially intestinal motility, and sleep health. However, little is known about the relationships between intestinal motility and sleep health. Therefore, the relationships among functional constipation (FC), irritable bowel syndrome (IBS) and sleep health in the population of middle aged women living in the Tokyo metropolitan area were investigated using the self-reported questionnaire. FC and IBS were defined by the Rome diagnostic criteria for the functional gastrointestinal disorders (Rome II). As a result, both the sleep health in FC and in IBS were significantly worse than in control, and there was a tendency for the sleep health in IBS to be worse compared with that in FC. Accordingly, these results suggest that there are relationships between conditions of intestinal motility, namely bowel habits, and sleep health. In order to improve sleep disturbance coming into question in recent years, the consideration of bowel habits also seems to be important.

    The present study was conducted to investigate the interactions in a pair for sleep health and sleep habits. A questionnaire was administered to daytime workers and their partners in a Tokyo metropolitan area, and healthy 249 pairs (20-59 years) were accepted for the analysis to find out risks in sleep health. Both wives and husbands whose partners showed higher risk scores in sleep health were in poor sleep health status. It was showed that sleep habits of husband affected sleep health of wife, but no effects of wife. The influences of husband's sleep were suggested to be predominant.

    The sleep disorder associated with pregnancy usually is biphasic, which typically begins with excessive sleepiness and develop to severe insomnia. The purpose of this study is to investigate into actual circumstances about sleep health, habits and symptoms of sleep disorders in pregnancy. Normal pregnant women (n=364, 29.7±4.2 years old) and healthy non-pregnant women (n=194,34.0±4.2 years old) living in the same area were asked the sleep-health used self-reported questionnaire with informed consent. There was no difference for sleep onset time between pregnant and non-pregnant women. Delayed rising time and prolonged sleep length in all trimesters of pregnancy were shown. The sleep maintenance and sleep initiating were disturbed in pregnant women. The prevalence of possible restless legs syndrome/possible periodic limb movement disorder was highest in 2 nd trimester (9.0%), however lower than it in the report of USA. Sleep bruxism was shown the highest prevalence in 1st trsis gravidarum disturbed sleep maintenance, sleep initiating and caused the deteriorated waking-up. This study showed that sleep was disturbed in 1 st trimester of pregnancy with high psychological anxiety, hyperemesis gravidarum. It was also suggested that the symptoms caused difficulty of sleep initiating, i. e. possible restless legs syndrome/possible periodic limb movement disorder, were appeared during all trimesters of pregnancy.imester.     東京都内の大学病院に勤務する看護師1111名に質問紙調査を行い、勤務形態の違いが睡眠・運動習慣・食生活に与える影響、健康関連QOLの低下や体重増加のリスク要因などについて検討した。結果、交替制勤務への従事や2交替・3交替という勤務形態の違いがQOLと体重増加に対して直接的に影響を与えていることを示唆するデータは得られなかった。しかし、QOLに対しては睡眠状態の悪化を通じて勤務形態が影響を与え、体重増加に対しては食生活の乱れを通じて勤務形態が影響を与えている可能性が示唆された。勤務形態別でQOLを比較すると3交替群よりも2交替群で低かった。

    The effects of simulating dawn lighting with an ordinary ceiling light on the quality of awakening were investigated. The subjects were healthy men, aged 24-27. After sleep for about 7 hours with polysomnographic recordings in a climatic chamber kept at a temperature of 25℃ and a relative humidity of 50%, each subject was awakened by an alarm. For the 30 minutes immediately before the subject was awakened, the illumination in the chamber was gradually increased, simulating the condition of waking up as the sun rises. Alpha attenuation test, measuring blood pressure and awakening feelings were conducted after awakening. The time of sleep stage 2 appearing during the 30 minute simulating dawn lighting was significantly less (p < 0.05; Wilcoxon test) than during typical waking conditions (i.e., waking up in the dark), while the time of stage W was higher (p = 0.0796). The mood after awakening in the simulating dawn lighting condition was significantly better (p < 0.05). There were no significant differences in the alpha attenuation coefficients and blood pressures after awakening between the two conditions. These results suggest that simulating dawn lighting arouses subjects to light sleep, which makes awakening less sudden and more pleasant. There was no evidence that the light sleep immediately before awakening decreased the subjects' cerebral cortex activity or sympathetic nervous activity after awakening.

    本研究では、児童期から青年期にかけての脳や身体、こころの発育に重要な役割を果たす睡眠を良好に確保するためのアプローチとして新しい睡眠教育パラダイムを構築し、有効性の高い睡眠健康支援を確立することを目的としている。本年度、本研究グループでは小中学校、高等学校と連携して対面形式やオンライン形式での睡眠教育を実施した。睡眠教育では、クイズ形式のパンフレット教材を活用しながら知識の獲得を促し、かつ睡眠改善に重要な目標行動を設定させ、知識に対応させながら行動変容を狙った。加えて、睡眠教育に児童生徒の保護者にも参加してもらい、学校と家庭で取り組みたい活動について周知を試みた。睡眠教育後は、保健だよりを通じて、各校で実施した調査結果と睡眠改善のポイントをまとめた内容を配布してもらい、睡眠教育に参加できなかった家庭に向けての情報提供も試みた。さらに、地域全体で子どもの睡眠健康を支える環境を整備すべく、地域住民や高齢者を対象とした睡眠教育も実施し、各家庭や家族全体で健康的な睡眠を確保することの重要性に関する情報提供も行った。この他にも、地域住民に向けたパンフレット教材や幼児の睡眠健康度を高めるための情報を盛り込んだ「あなたの睡眠健康度診断」というHPを作成した。現在は、上述の睡眠教育の効果を評価するための解析を実施している。次年度は、睡眠教育に対する児童生徒の関心や態度を把握するための準備を進めている。

    The characteristics of the circadian rhythm by age group, that is, by life stage, are clarified in humans and mice. The life stage is divided into school children, adults and middle-aged and elderly. We investigated the importance of approaches of chrono-nutrition, chrono-exercise, and chrono-sleep on health science. The importance of regularity of feeding and sleep rhythms in the learning performance of school children and adolescents. For protection of adult obesity and hypertension, equal meal ratio of breakfast, lunch and dinner and reduction of carbohydrate at dinner, and intake of KCL through vegetables, are recommended, respectively. In addition, it was shown that more protein intake at breakfast was necessary for preventing muscle atrophy like sarcopenia in aging peoples.

    不眠症患者の日中機能評価と睡眠薬依存形成の原因を解明するために、治療に用いられる睡眠薬の離脱時の症状を評価するスケール(benzdiazepine hypnotics withdrawal symptom scale; BHWSS)を作成し、benzodiazepine受容体アゴニスト長期使用時における離脱症状の発現要因を調べた。その結果、睡眠薬服用者の約8%に離脱症状が生じること、服薬コンプライアンス不良が離脱症状の発現と最も強く関連することを明らかにした。
    不眠症者については、症状特性と生活の質(QOL)の関係、ならびにプレゼンティーイズム、アブセンティーイズムとの関係を調べ、ブルーカラー、ホワイトカラーの間で比較した。これにより、入眠障害と睡眠維持障害の存在がプレゼンティーズム、アブセンティーズムを悪化させること、両者の重複が、これらの指標のさらなる増悪につながること、対象者の経済状況が関与することが明らかになった。また、不眠症状の長期化は身体的QOLの悪化につながること、精神的QOLは不眠症直後に悪化すること、睡眠薬の服用動機として入眠障害の関与が大きいことが明らかになった。
    閉塞性睡眠時無呼吸(OSA)について、健診データの身体指標(肥満度、首周囲径、ウエスト周囲径)を用いたスクリーニングスケールを作成し、その陽性者における医療機関受診実態を、健保組合の医療レセプトデータを用いて解析し、受診と関連する心理社会学的要因を検索した。これにより、独居であること、ブルーカラーであること、合併症がないこと、低年齢であることが非受診の関連要因であるという結果が得られた。

    This study was set out to study the prevalence of delayed sleep phase disorder (DSWPD), impact of its morbidity on daytime function and associated factor of the disorder.After identifying the validity and reliability of the Japanese version of Biological Rhythms Interview of assessment (BRIAN-J)in a preliminary study, we conducted web-based questionnaire study about sleep hygiene related behaviors and the prevalence of DSWPD by using BRIAN-J targeting young generation aged 15-30 years. As a result, 4.3 % of the subject population was identified as having DSWPD, and individuals affected with DSWPD were thought to have deteriorated scores of work productivity, depression and health related quality of life. Moreover, the presence of DSWPD was thought to be associated with being student, having currently treated disease and longer liquid crystal display (LCD) viewing time.
    Our result could emphasize the importance of keeping better sleep hygiene for preventing the development of DSWPD.

    Misalignment between an individual’s circadian phase and his/her environment due to social imposition is called social jetlag (SJL). This study aimed to elucidate the percentage of SJL in a large Japanese population using the Japanese version of the Munich ChronoType Questionnaire (MCTQ). A total of 10,000 participants responded to a web-based cross-sectional survey, and the final sample comprised 3,708 participants. SJL was measured by subtracting each participant’s midpoint of sleep on workdays from their midpoint of sleep on free days. The average [SD] absolute SJL (SJLabs) value was 0.91 [0.89] hours, and the percentage of participants with SLJabs of more than 1 hour was 40.1%. Our data also showed that SJLabs becomes progressively smaller with advancing age. A considerable number of participants of our Japanese population showed differences in sleep behavior between workdays and free days.

    The aim of the study was to examine the relationship between phenotype of insomnia and sleep-wake rhythms, anxiety, and depression. (1) The results showed that individuals with sleep onset insomnia significantly had more eveningness chronotype than good sleepers, and individuals with sleep-maintenance/early awakening insomnia significanlty had more morningness chronotype than good sleepers.
    In anxiety and depressive symptoms, individuals with sleep onset insomnia and that with mixed phenotypes were highest score in all groups. (2) We developed cognitive behavioral therapy for regulating sleep-wake rhythms (CBT-RSW) and conducted an open trial of CBT-RSW for adult with neurodevelopmental disorders. The results showed that insomnia severity, social disabilities, and anxiety/depressive symptoms improved and effect sizes were small to large.

    The aim of this study was to elucidate the level of daytime sleepiness among children and adolescents using the Japanese version of the Pediatric Daytime Sleepiness Scale (PDSS-J). After the linguistic validation of the PDSS-J with a multi-step translation methodology, we conducted a psychometric validation. Multivariate logistic regression analysis revealed that both short sleep duration and social jetlag were identified as factors associated with daytime sleepiness. PDSS-J scores were significantly higher in the group with large social jetlag with or without sufficient sleep duration than in the group with sufficient sleep duration and small social jetlag. The PDSS-J is an important tool for assessing daytime sleepiness, given its ease of administration and robust psychometric properties. The impact of not only sleep loss but also social jetlag on daytime sleepiness among school-aged children and adolescents must be fully taken into account.

    The prevalence of hypnotic dependence was 7.9 percent in psychiatric outpatients taking hypnotics. Hypnotic dependence was significantly associated with younger age, higher total Pittsburgh Sleep Quality Index (PSQI) score, and higher daily dose of hypnotics. Even though patients with hypnotic dependence frequently suffered from subjective side effects and had greater sleep difficulty, they continue taking high dose of hypnotics. The prevalence of sleep-related eating disorder (SRED) was relatively high with 8.4 percent in psychiatric outpatients taking hypnotics. The occurrence of SRED was significantly associated with younger age, higher bedtime diazepam-equivalent dose of hypnotics and combination with antipsychotic drugs. It is suggested that the age of onset of hypnotic induced secondary SRED was high compared with other SRED, and hypnotic induced secondary SRED was characterized by lack memory of symptoms.

    To clarify the significance of melatonin secretion under intensive care conditions, we investigated melatonin secretion profiles and sleep parameters of 23 patients just after admission to the coronary care unit (CCU) and 19 age-matched controls. Sleep parameters were evaluated by actigraphy, and melatonin secretion was assessed by measuring the urinary 6-sulphatoxy melatonin (6-SMT). 6-SMT secretion was lower and nocturnal sleep parameters were less satisfactory in the subjects than those in the controls, and there were positive correlations between these variables, particularly in the subject patients. The lowered melatonin secretion might be involved in the mechanism of insomnia in CCU patients.

    Series of our study revealed that longer elapsed time, shorter sleep duration (less than 6hours) and existence of habitual snoring/nocturnal apnea episodes were independently associated with the experience of vehicular accidents among general drivers. This finding suggest that early treatment of sleep disordered breathing (SDB) and securing sufficient amount of nocturnal sleep is necessary to prevent sleepiness related traffic accidents. Our study also showed that among occupational drivers the prevalence of shift work related sleepiness is higher than that of SDB related sleepiness. However, they seem not to take counter measure effective for reducing their sleepiness even when they are in a sleepy condition because they do not have enough time for taking a rest possibly due to their workload.

    Sleep-related eating disorder (SRED) is a behavioral disorder in which recurrent episodes of eating and drinking occur during the main sleep episode. The Munich Parasomnia Screening (MUPS) was designed as a screening instrument to assess the occurrence and frequency of nocturnal behaviors and events. We developed a Japanese version of the MUPS in order to assess the lifetime prevalence and current frequency of SRED. The anonymous questionnaire survey revealed that lifetime prevalence of SRED in the subject population was 5.5%. From the study investigating the consecutive patients with SRED, there are different clinical features between primary and secondary SRED.

    Obesity and craniofacial abnormalities are common features of patients with Obstructive Sleep Apnea(OSA). Anatomical balance between the size of the craniofacial rigid enclosure(formed by the dentitions, cervical vertebrae, maxilla, and mandible) and the amount of soft tissue(e. g. tongue) influences the upper airway space and is involved in the pathogenesis of OSA. In this study, we investigated whether this anatomical balance was different between races in OSA people. Blind measurements of tongue cross-sectional area and craniofacial dimensions were performed two dimensionally through lateral cephalograms in 76 Japanese patients with OSA and 74 Caucasian OSA subjects after matching gender(male), age, and apnea hypopnea index. Sagittal tongue size, sagittal maxillomandibular dimensions, and body mass index(BMI) were compared between the groups. While BMI was significantly higher in Caucasians than in Japanese patients, tongue size and maxillomandibular dimensions were not different between the races. These findings suggest that anatomical balance does not differ between Japanese OSA people and Caucasian OSA persons under the same OSA severity. This may partly account for the reason why prevalence of OSA are almost the same between Japanese and Caucasians.

    This series of studies have been made to investigate the differences and similarities of clinical characteristics and quality of life among CNS hypersomnias (narcolepsy related disorders). As a result, it was shown that nocturnal sleep disruption is the specific symptom of narcolepsy with cataplexy among the CNS hypersomnias. The decrease in CSF orexin secretion was significantly associated with the increased propensity of sleep induction and appearance of stage REM, both of which are the cardinal feature of narcolepsy. Based on this, CNS hypersomnias showed difference in both the severity of hypersomnia and the treatment response. However, of note, quality of life the patients manifested on the score of SF36 seemed to be deteriorated depending not on the subjective severity of hypersomnia but on the degree of the lack of familial and social support.

    健康成人20名(21.7±0.92歳)を対象として,認知機能に関する実験を実施した.これまで報告者が蓄積してきた実験手続きやスケジュールを駆使し,剰余変数の混入を防ぎ,厳密な統制条件下で行った。実験被験者には本研究の実験内容および手続きを十分に説明し,インフォームドコンセントを得た。各被験者に対し,実験3日前より生活統制を行った。通常の就床時刻をCircadian time 0時とし,規則正しい生活を送るよう指示した。非利き腕にアクチグラフを装着させ,生体リズムの統制を厳密に確認した。実験前日は自宅にて通常の睡眠をとらせるControl条件と,睡眠不足状態をシミュレートすることを目的とし90分間の部分断眠を課すDeprivation条件をカウンターバランスで配置した。翌CT9時に実験室に来室させ,脳波等を装着した上で以下のテストバッテリーを使用し,前頭連合野機能への影響を測定した。テストバッテリーは,(1)visual analog scaleを用いた眠気や意欲に関する主観調査,(2)安静時開眼および閉眼時脳波測定,(3)事象関連電位P300測定,(4)psychomotor vigilance task(数字加算課題,作業記憶課題,記憶操作課題(セマンティックプライム),英数字検出課題,go/no-go課題)とした。部分的睡眠遮断がどの前頭連合野機能へより強く影響を及ぼしているか,回復過程を用いて確認するため,CT10時より20分間安静仰臥位を保たせた後,再度テストバッテリーを施行した。
    その結果,主観的眠気(KSS)は,通常睡眠後の午前セッションでは,通常睡眠午後・部分断眠後午前・午後のセッションに比べて,有意にKSS得点が低く,眠気が弱かった。生理的眠気(AAT)は,通常睡眠後ならびに部分断眠負荷後どちらにおいても,午後のセッションは午前のセッションに比べて,有意に生理的眠気が強かった。事象関連電位については,午前の測定では部分断眠を負荷した場合,P300頂点潜時は延長し,振幅が低下したが,午後の測定では部分断眠の影響は見られなかった。若年者を対象とした今回の実験の結果,午前の時間帯では,部分断眠の影響で,脳内情報処理速度は遅延し,処理能力が悪化した。午後の時間帯では,通常睡眠の場合も部分断眠負荷条件と同様に,主観的・生理的眠気の増強と,脳内情報処理の悪化が認められた。部分断眠によって,ワーキングメモリの機能が低下する傾向がみられた。日常よく経験するような90分程度の部分断眠(睡眠不足)であっても,記憶の引き出しの妨害が生じている可能性がある。継続的な部分断眠(慢性的な睡眠不足)がワーキングメモリに及ぼす影響について今後検討する必要がある。部分断眠の負荷およびサーカディアンリズムによって生じる眠気は,記憶・学習能力や注意集中力に影響を及ぼし,判断ミスや事故を引き起こす原因となる。日常生活においては,眠気を軽視しがちであるが,脳機能の観点から睡眠の役割を再認識する必要がある。

    入眠前覚醒の時間的長さと質,サーカディアンリズムの位相など入眠過程に影響する諸要因を極力統制した上で,夜間睡眠・日中仮眠の生理的入眠過程を検討した。大学生418名(男性235名,助成183名,21.1±8.5歳)を対象として,性格特性に関しては,矢田部・ギルフォード性格検査を,行動特性に関してはラザルス式ストレスコーピングインベントリーを,精神健康に関しては自己評価式抑うつ尺度および精神健康調査票を,生活・睡眠習慣に関しては睡眠健康調査票を実施した。睡眠健康調査票には,入眠困難性に関する3質問項目を追加した。この調査から,入眠潜時が20分未満で寝つきが良い「主観的入眠容易群」と入眠潜時が30分以上で寝つきが悪い「主観的入眠困難群」を抽出した。主観的入眠容易群は189名(45.1%),主観的入眠困難群は26名(6.2%)であった。両群から候補者をランダムにサンプリングし,研究内容を十分に説明し,研究協力の同意が得られた各群8名のボランティアを被験者とした。被験者には実験一週間前よりアクチグラフを装着し,睡眠・覚醒リズムを一定にし,日中運動量,食事回数・時刻,禁酒等を含む生活統制を行った。実験は2条件で構成し,カウンターバランスをとって実施した。両条件とも1夜の夜間睡眠と翌日日中の仮眠からなり,実験室での夜間就床時刻・起床時刻は,各被験者の習慣的就床時刻・起床時刻を使用した。日中仮眠の消灯時刻は前夜の就床時刻より15時間後(CT15)とし,消灯40分後に呼名点灯し仮眠を終了した。日中仮眠前に40分,2,500lxの高照度光照射を行うBright light (BL)条件と10lxの室内灯下で過ごさせるDim light (DL)条件を設けた。脳波,眼電図,筋電図を同時記録し,入眠過程を5段階に分類し10秒ごとに判定した。
    その結果,主観的入眠困難群は主観的入眠容易群に比べて,夜間睡眠・日中仮眠とも入眠過程は延長しており,特にα並が消失してθ波期に至る入眠初期段階の遅延が顕著であった。入眠困難性を示した被験者の生理的特性を検討したところ,夜間消灯後1時間のHRが高く,交感神経系活動の高いことが示唆された。心理的特性については,主観的入眠困難群は主観的入眠容易群に比べ,神経質で抑うつ感や劣等感が強いこと,ストレスにうまく対処できず他者に原因や責任を転嫁する傾向が示され,主観的および客観的に入眠困難を呈する被験者は,特有の心理的特性をもつことが明らかとなった。さらにBL条件での日中仮眠時人眠過程は,主観的入眠容易群では延長し,主観的入眠困難群では逆に短縮した。主観的入眠困難群では入眠前の脳の活動性は上昇しており,もともと入眠しづらい状況がつくりだされている。こうした睡眠傾向の低い被験者に対して,BL条件の適用は脳の活動性を一過性に過活動化させ,その結果生じた急激なリバウンドによる低下が入眠を促進させたものと考えられた。
