    Sarcopenia is characterized by a gradual slowing of movement due to loss of muscle mass and quality, decreased power and strength, increased risk of injury from falls, and often weakness. This review will focus on recent research trends in nutritional and pharmacological approaches to controlling sarcopenia. Because nutritional studies in humans are fairly limited, this paper includes many results from nutritional studies in mammals. The combination of resistance training with supplements containing amino acids is the gold standard for preventing sarcopenia. Amino acid (HMB) supplementation alone has no significant effect on muscle strength or muscle mass in sarcopenia, but the combination of HMB and exercise (whole body vibration stimulation) is likely to be effective. Tea catechins, soy isoflavones, and ursolic acid are interesting candidates for reducing sarcopenia, but both more detailed basic research on this treatment and clinical studies in humans are needed. Vitamin D supplementation has been shown not to improve sarcopenia in elderly individuals who are not vitamin D-deficient. Myostatin inhibitory drugs have been tried in many neuromuscular diseases, but increases in muscle mass and strength are less likely to be expected. Validation of myostatin inhibitory antibodies in patients with sarcopenia has been positive, but excessive expectations are not warranted.

    PURPOSE: This study attempted to clarify the relationships between marathon time and monthly training volume, training frequency and the longest (LRD) or average running distance per workout (ARD), as well as their interactions. METHODS: Male recreational runners (n = 587) participating in the Hokkaido Marathon 2017 completed a questionnaire before the race; of these, 494 finished the race. We assessed age, running career, body height, body weight, body mass index (BMI), monthly training volume, training frequency, the LRD and the ARD. These indicators were each divided into 4 or 5 homogeneous subgroups to determine whether the other indicators in each subgroup predicted marathon time. RESULTS: In the training frequency subgroups, there were significant correlations between monthly training volume, the LRD or the ARD and marathon time, except for the subgroup that trained 2 times per week or less; in this subgroup, the relationship between the ARD and marathon time was not significant. In all monthly training volume subgroups, there were no significant relationships between training frequency, the LRD or the ARD and marathon time. In the ≥ 21 km LRD and ≥ 10 km ARD subgroups, there were significant correlations between monthly training volume and marathon time (all P < 0.01); these correlations were not significant in the 1-20 km LRD and < 10 km ARD subgroups. CONCLUSION: These results indicate that monthly training volume is the most important factor in predicting marathon time and that the influence of monthly training volume is only significant if the running distance per workout exceeded a certain level.

  • Association of Oral Function and Dysphagia with Frailty and Sarcopenia in Community-Dwelling Older Adults: A Systematic Review and Meta-Analysis

    Studies investigating the associations of oral function and dysphagia with frailty and sarcopenia in community-dwelling older adults are increasing; however, they have not been systematically summarized. We conducted a systematic review to investigate these associations. We searched electronic databases and synthesized relevant data using conventional (frequentist-style) and Bayesian meta-analyses. Twenty-four studies were found to be eligible for our review, including 20 cross-sectional and four prospective cohort studies. Older adults with frailty or sarcopenia had lower tongue pressure, according to the results of conventional meta-analysis (mean difference [95% confidence interval or credible interval]: −6.80 kPa [−10.22 to −3.38] for frailty and −5.40 kPa [−6.62 to −4.17] for sarcopenia) and Bayesian meta-analysis (−6.90 kPa [−9.0 to −4.8] for frailty, −5.35 kPa [−6.78 to −3.89] for sarcopenia). People with frailty had a higher odds ratio (OR) for dysphagia according to the results of conventional meta-analysis (3.99 [2.17 to 7.32]) and Bayesian meta-analysis (1.38 [0.77 to 1.98]). However, the results were inconclusive for people with sarcopenia. A prospective association could not be determined because of the lack of information and the limited number of studies. Decreased oral function and dysphagia can be important characteristics of frailty and sarcopenia in community-dwelling older adults.

    In patients with heart failure (HF), it is important to perform exercise therapy with a focus on the pathophysiology of skeletal muscle. Patients with HF have multiple clinical symptoms due to cardiac dysfunction. Recent studies demonstrated the mechanism and treatment strategy for HF, and multiple signaling pathways involved in HF result in reduced exercise capacity and skeletal muscle mass. On the other hand, exercise therapy for HF is known to inhibit the inflammatory cytokines and neurohumoral factors, and increase muscle mass. Therefore, in this chapter, we discuss the importance of exercise therapy for HF, with a focus on the pathophysiology of skeletal muscle.

    Presenilin-1 and nicastrin, two components of gamma-secretase associated with Alzheimer's disease plaques, are present in the synapses of the brain and in various peripheral organs, including skeletal muscle. In the present study, we examined the expression pattern of presenilin-1 and nicastrin in normal and denervated hindlimb muscles of the rat. Using immunohistochemical approaches, we found that presenilin-1 and AChRalpha was co-localized at the neuromuscular junction in the normal skeletal muscles of rats. The immunoreactivities of both presenilin-1 and nicastrin were also observed at the sarcolemma of muscle fibers. We discovered that presenilin-1 mRNA and its protein are upregulated after denervation of the soleus and tibialis anterior muscles. Furthermore, clear co-localization between presenilin-1 and DAPI, but not nicastrin, was noted in several myonuclei in the denervated muscles. We recognized a few fibers possessing both ubiquitin and presenilin-1 protein in the cytosol. The amount of presenilin-1 in the nucleus and membrane fraction was more abundantly expressed in the denervated muscle fibers. In contrast, no significant difference in the nicastrin protein level was observed between normal and denervated muscle fibers. These data suggest that enhanced presenilin-1 protein may play a role in the degeneration and regeneration of skeletal muscle.

    骨格筋は様々な因子により調節を受け、オートファジー機能不全によりサルコペニア (加齢性筋減弱症)が促進される可能性が高い (Sakuma K, et al., p62/SQSTM1 but not LC3 is accumulated in sarcopenia muscle of mice. Journal of Cachexia, Sarcopenia, and Muscle. 2016)。オートファジー機構を強力に阻害するものとして、最近注目を集めている物質がRubiconである (Nakamura S et al., Nature Commun 10: 847, 2019)。このRubiconはBeclin-1と結合することで、ライソゾームとオートファゴソームの合体を阻止する可能性がある。本研究では、加齢筋におけるRubiconとBeclin-1の変化について、蛍光免疫組織染色を用いて調べた。実験動物には若齢 (3ヶ月齢)と高齢 (24ヶ月齢)のC57Black/6J雄マウスを用いた。筋凍結横断切片 (8 μm)を用いた蛍光染色から、加齢筋におけるRubiconの免疫活性亢進が認められたが、Beclin-1の免疫活性については加齢にともなう明らかな変化が認められなかった。またRubiconの免疫活性亢進は、除神経を施した萎縮骨格筋においても同様に確認された。加齢筋におけるRubiconの活性亢進の場所は、筋細胞の細胞膜や個々の筋細胞質ではなく、細胞内にパーティクル (小片)上に散在していた。分画したタンパク質を用いたWestern blot法により、Rubiconタンパク質の加齢筋における変化のさらなる検証を行う予定である。また他のオートファジー関連物質であるアペリンについても、徐々に解析を進める予定である。

    Autophagy dysfunction occurs in sarcopenia (age-related myopathy). Using age-related and denervated muscles, we compared the manner of adaptive changes of mitophagy (mitochondrial autophagy: Parkin) and chaperone-mediated autophagy (Bag3) and aimed to develop a shorter, more convenient model of autophagy dysfunction in sarcopenia. Our result shows that the changes in autophagy-related modulation of ageing muscles occurs in the denervated muscles. It was found to be partially reproducible.

    This study examined the interactions between intestine barrier function, microbiota and metabolic/secretory function of skeletal muscle. We found that impairment of intestine barrier function caused glucose intolerance with muscle metabolic dysfunction in experimental mouse models. In addition, in the mice transplanted with microbiota from trained mice, metabolic/secretory factors in skeletal muscle were enhanced. On the other hand, in the mice with impaired muscle metabolism, myokine secretory- and intestinal barrier- functions were suppressed. These results support the concept of organ communication between skeletal muscle and gut.

    Using senescent mice (24-month-old), I investigated the adaptive changes in endoplasmic reticulum stress-linked molecules in skeletal muscle. Although we observed the marked increase in the amount of IRE-1a and PKCtheta protein in cytosol of aged muscle fibers, these expressing position are not equal at those position of autophagy-linked molecules. Long-term low-protein-diet did not attenuate sarcopenic symptom. Supplementation with resveratrol, which decrease endoplasmic reticulum stress, attenuated muscle atrophy after denervation in mice.

    SPARC is one of myokines, bioactive proteins secreted from skeletal muscle. The level of circulating SPARC is increased in response to exercise, and contributes to prevention of colon carcinogenesis by habitual exercise.
    Our results showed that SPARC directly affects the lesion site of the colon, and prevents carcinogenesis. In addition, the SPARC could increase metabolic capacity of skeletal muscle itself, leading to improved whole body metabolism, which also contributes to the suppression of colon carcinogenesis. The increase of circulating SPARC was found in both healthy subjects and metabolic patients. In addition, dietary approach has been suggested to accelerate the secretion of SPARC efficiently.

    In the present study we aimed to measure exhaled carbon monoxide (CO) during and after exercise in smokers and non-smokers. We could observe that exhaled carbon oxide increased relatve to exercise intensity and decreased rapidly after exercise. The amounts of CO exhalation in smokers were significantly larger than those of non-smokers. We also attempted to develop a prototype model to estimate carboxyhemoglobin concentrations (COHb) transcutaneously in the blood using multi-wavelength optical sources. The COHb could be continuously measured by a prototype model, however problems relevant to the stability and resolution still remained. Furthermore, we also measured the expression of heme oxygenase-1 and -2 which are the enzymes producing CO in mice, and found that the expression of these enzymes in the skeletal muscles increased with acute exercise and strength training.

Although voluntary exercise attenuated the sarcopenic symptom, there is no significant change in autophagy-linked molecule.

    筋発生過程だけではなく成熟した哺乳動物の筋肥大においてもRhoA-SRF(Serum response factor)経路は重要な役割をする。ラットの足底筋に対する代償性過負荷(Mechanical 0verloading)は筋肥大をもたらすが、このとき分化期の衛星細胞(satellite ce11)にRhoAおよびSRFが発現することを我々は報告してきた(Sakuma K et al.,Histochem Cell Biol 119:149-160,2003)。SRFは、MRTF-A(myocardin-related transcription factor A)やMRTF-Bと複合体を形成し、転写促進に働くことが最近わかってきた。昨年度の我々の研究は、加齢にともない骨格筋中のSRFおよびMRTF-A &-Bの発現が減少し、これらの物質を調節するRhoAやその阻害物質RhoGDIは有意に増加することを示唆した。今年度は、細胞分画法と蛍光免疫組織染色を用いて、加齢にともなうSRF関連物質の変化をより詳細に調べることを目的とした。実験動物にはC57Black/6Jマウス(3ヶ月と24ヶ月齢)を用い、大腿四頭筋を分析に用いた。各マウスの筋から、細胞分画キットにより、核分画、細胞質分画、膜分画、細胞骨格分画にタンパク質を分離した。また7μmの凍結横断切片を作成し、蛍光免疫組織染色法によりSRFの局在様相を調べた。加齢マウスの筋において、核分画と細胞質分画の両方でSRF蛋白量は有意に減少した。MRTF-A蛋白については、核分画においてのみ加齢にともなう減少が認められた。一方、RhoA蛋白については細胞膜と膜分画で、RhoGDI蛋白については核、細胞膜、膜分画の3ヶ所において加齢にともなう有意な増加が認められた。若齢筋では、CD34で識別される多くの衛星細胞ならびに筋線維の細胞質にSRFの免疫活性が認められたものの、加齢筋では、衛星細胞や筋線維の細胞質にSRF活性がみられなかった。筋細胞の骨格を形成するアクチン、ミオシン軽鎖、デスミンといった物質は、SRFおよびMRTFの核内移動で転写促進される可能性が高い。加齢にともなう骨格筋でのSRF-MRTFの顕著な減少が、筋細胞の萎縮を誘導する可能性が示唆された。

    Neuregulin(NRG) belongs to the family of multipotent growth-promoting proteins, and mainly present at neuromuscular junctions(NMJs). Furthermore, NRGs have been shown to be one of the most significant factors to accumulate AChR at NMJs. While several studies have been conducted on the roles of NRGs in muscle fiber development, little is known about their roles in muscle regeneration. We hypothesize that NRGs play important roles in NMJ reconstruction and in the regeneration of muscle fibers. There has been no study that tried to prove this hypothesis. Therefore, we investigated changes in NRG expression and the relationship with muscle satellite cells which were one of the significant factors of muscle regeneration in damaged muscles. Besides, we were also interested in the NRG expression of the motoneuron which connects target organs and central nerve, and we examined the simultaneous changes in NRG expression in the spinal cord.
    We used 10 week adult Wistar rats. Muscle regeneration of tibialis anterior (TA) muscle was induced in one leg by an intramuscular injection of bupivacaine hydrochloride. The TA muscles of injected legs and spinal cords dissected at 1, 2, 4, 6, 10 and 14 days post surgery.
    Western blot shows that NRG proteins in the regenerating muscles decrease on the 1st day and increase after the 4th day up to 2 weeks. NRG mRNA in the muscle increases from 2 to 6 post-surgical days. The cellular localizations of AChR-a and NRG are shown, respectively. The expression of NRG was detected in some of quiescent satellite cells expressing c-Met. Several c-Met negative mononuclear cells existing near the membrane of muscle fibers also possessed NRG immunoreactivity in unoperated TA muscle of rats. At 2 days post-injection, the cellular localizations of PCNA and NRG are shown, respectively. In addition, BrdU-positive satellite cells also possessed a NRG protein in the regenerating muscle at 2 days post surgery.
    At 6days after damage, marked NRG and AChR-a immunoreactivities were observed in the regenerating NMJs. Furthermore, NRG expression was also detected at the myogenin- positive satellite cells respectively.
    Western blot analysis shows the gradual increase in NRG proteins in the spinal cord after the first day following damage. However NRG mRNA in the spinal cord is not influenced after muscle damage.
    In conclusion, we showed that when muscle was damaged, NRG proteins increased in the regenerating muscles. Combined with the change of NRG mRNA, it seems that NRG promote regeneration in the damaged muscle fibers. Secondly, we showed that NRG immunoreactivity was detected through the proliferation and differentiation stages using immunofluorescence. Thirdly, when muscles were damaged, motoneurons seemed to be influenced by peripheral damage. Western blotting demonstrated that NRG proteins increased in the spinal cord with time, following damage. Interestingly, we did not observe significant changes in NRG mRNA. We indicate a possibility that the NRG protein was transported retrogradely to the spinal cord in vivo.

    Orexins (OXs) have been known as a hypothalamic regulating factors in the defense response, so called "fight or flight" response, which is characterized by a concomitant activation in arousal, HPA-axis, sympathetic nerve system and alpha-motoneurons (α MN) facilitation. These physiological reactions are orchestrated to involved in the regulation of physical activity in "fight or flight" response. We hypothesized that OXs would simultaneously be involved in the αMN facilitation and HPA-axis activation. To address this, we examined whether OX-1R (type-1-receptor) expresses on αMN, connecting to hindlimb soleus and extensor digitorum longus (EDL) muscle, respectively. As a result, almost all αMN to their respective muscles had OX-1R. To identify whether OXs were involved in the αMN-facilitation via OX-1R in lumbar, we examined the escape swimming response to stress provoked by immersing rats into water after pretreatment of OX-1R antagonist administration. The inhibition of OXs action via OX-1R on αMN decreased the escape swimming activity. Under the same stress, neurons in PVN and LHA were activated. ACTH release which promoted by water immersion stress was suppressed by pretreatment of OX-2R (type-2-receptor) antagonist administration.
    Plasma concentrations of OXs were higher in neonates and infant during decelopment, and OX-receptors express on skeletal muscles and αMNs. It leads us hypothesized that OXs would mediate growth and development of skeletal muscle through both humoral and neuronal pathways. First, we examined hind limb muscular weight between prepro-OX knock-out (KO) mice and littermate wild-type (Wt) mice (10-50 weeks-old), and found that both plantaris and EDL muscles in KO mice atrophied significantly. In addition, we investigated the effects of OXs on the proliferation and defferentation of myoblasts in vitro culture system, and found that OXs accelerated the proliferation of myoblasts.
    We found that OXs plays dual roles for facilitating αMN and the HPA-axis through their specific receptors. Again, OXs play a potential role to promote myoblasts' proliferation in vitro. These results underscore orexin's role in muscular development and exercise-related functions, leading us to examine the role of orexin on some response and adaptation of body to exercise. (349)

    加齢にともない筋細胞が萎縮、脱落するために、筋力が著しく衰える。この老化にともなう筋萎縮に筋肥大抑制遺伝子(myostatin)が関与しているかどうかについて我々は研究を進めている。今年度は、骨格筋内のmyostatin、その下流で働くSmad3、ならびにmyostatinの働きを抑制するfollistatinのmRNA発現量が加齢にともない変化するかどうかについてRT-PCR法により調べた。実験には生後3ヶ月齢と24ヶ月齢のC57 Black/6J系雄マウスを用い、各マウスから大腿四頭筋と上腕三頭筋を摘出した。RNAの抽出はセパゾール(ナカライ)により行い、各バンドを定量するためのコントロールとして18SのリボゾーマルRNAを用いた。3ヶ月齢マウスの大腿四頭筋においてfollistatin、myostatinおよびSmad3のmRNAが確認できたが、加齢による有意な変化はみられなかった。一方、上腕三頭筋では、加齢にともないfollistatin mRNAが有意に減少し、myostatinとSmad3のmRNA発現量には有意な変化がみられなかった。これに対してmyostatinにより発現が調節されるMyoD(筋分化促進因子)は、両方の筋において加齢にともない著しく増加した。以上のことから、上腕三頭筋ではmyostatinの働きを抑制するfollistatinが少なくなることで、結果的にmyostatinが筋萎縮を誘導する可能性が示唆された。

    加齢にともない脳内の神経細胞が変性、脱落する。この加齢にともなう神経細胞死に対して、運動に抑制効果があるかどうかについて我々は研究を進めている。今年度は、持久性トレーニングが、脳内のアルツハイマー型痴呆関連物質[アミロイド前駆体蛋白(APP),presenilin-1,リン酸化タウ(PHF-tau)]の蛋白量を変化させるかどうかについて、免疫組織化学的手法を用いて調べた。Wistar系の雄ラットを用い、生後4週齢時から10週間の持久性走トレーニング(速度20m/min、1日60分、頻度週に5回)を施した。トレーニング終了後、ラットの大脳、小脳,脊髄を摘出し、各種蛋白の発現量を安静群(Control)と比較検討した。大脳において、APP, presenilin-1とも海馬のCA1,CA3領域の錐体細胞、歯状回の顆粒細胞、さらには大脳皮質の錐体細胞層に免疫活性が認められた。しかしながら持久性トレーニングによる有意な免疫活性の変化は認められなかった。小脳においては、APP、Presenilin-1とも顆粒細胞、プルキニエ細胞に発現が認められたが、定期的な運動による分布様相の変化はみられなかった。以上のことから若齢のラットにおいて、持久性トレーニングは中枢神経系におけるアルツハイマー型痴呆関連物質に量的な変化を引き起こさないことがわかった。

    一過性の運動が、脳内の神経栄養因子の受容体を活性化するかどうかについて調べた。Wistar系の雄ラット(6週齢)を用い、走運動は5m/分のスピードから開始し,5分毎に5m/分づつ漸増、20m/分になった時点から30分間の運動を行った(合計45分間)。運動終了後、30分,1,2,4,8,12,24時間後にラットの大脳、小脳、脊髄を摘出した。免疫沈降には組織サンプルを160-600μg使用し、TrkB、TrkCに対する抗体はそれぞれ0.5μg,1.0μg用いた。免疫沈降物を電気泳動した後にニトロセルロース膜に転写し、リン酸化チロシンを認識する抗体で神経栄養因子受容体の活性化の有無を調べた。その結果、大脳、小脳において運動1-12時間後までTrkB(BDNFとNT-4の受容体)のリン酸化が認められた。一方脊髄においては、運動後30分からすでにTrkBが活性化しており、運動後24時間が経過してもそのリン酸化が確認された。また大脳においてTrkC(NT-3の受容体)の活性化が、運動4,8,12時間後に確認された。小脳におけるTrkCの活性化は運動1-8時間後に認められたが、運動後の脊髄におけるTrkCのリン酸化はみられなかった。以上のことから、運動は神経栄養因子の受容体を活性化することで、脳内の神経細胞の生存・維持を促す可能性がある。加齢にともなう神経細胞の変性・脱落にも運動の効果が期待できるかもしれない。
